Home My WebLink AboutContract 50006-A4 CSC No. 50006-A4 AMENDMENT No. 4 TO CITY SECRETARY CONTRACT VMIERJUS, the City of Fort Worth (CITY) and Plummer Associates, Inc. , (ENGINEER) made and entered into City Secretary Contract No . 50006, (the CONTRACT) which was authorized by M&C C- 28464 on the 7th day of November, 2017 in the amount of $688, 037 . 00; and VM ERFAS, the CONTRACT was subsequently revised by: Amendment No. 1 in the amount $16, 005 . 00 administratively authorized on July 31 , 2018 , Amendment No . 2 in the amount of $54 , 039 . 00 administratively authorized on November 19, 2018 and Amendment No . 3 in the amount of $393, 152 . 00 authorized by M&C C-29090 on April 9, 2019; and VVUEREAS, the CONTRACT involves engineering services for the following project: North Holly Water Treatment Plant Sedimentation Basin Sludge Removal System Replacement; and VVHEltrAS, it has become necessary to execute Amendment No. 4 to the CONTRACT to include an increased scope of work and revised fee. I OVY THEREFORE CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1 . Article I of the CONTRACT is amended to include the additional engineering services specified in the proposal attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $6,443 . 00 . 2 . Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall be an amount of $1 ,157 , 676 . 00 . 3 . Project Manager City of Fort Worth 200 Texas Street Fort Worth,Texas 76102-4397 Re: North Holly Water Treatment Plant Sedimentation Basin Sludge Removal System Replacement (City Project No. 02328) Contract Amendment No. 4 Dear Ms.Johnson: The purpose of this letter is to request authorization for$6,443 in Special Services to update the CT study for the North Holly Water Treatment Plant.Amendment No.4 adds the following new scope items: • The Engineer will prepare a disinfection contact-time (CT)study worksheet for the North Holly Water Treatment Plant to reflect the changes made to disinfection zone 8 (D8) as part of the Sedimentation Basin Sludge Removal System Replacement project; • The Engineer will utilize the existing CT study(as approved by the Texas Commission on Environmental Quality in a letter dated July 9, 2014)for selecting the inputs for other disinfection zones and baffling factor for zone D8 (as established by previous tracer study*); • The Engineer will submit the CT study worksheet to the City for review; • The Engineer will address City comments and submit a cover letter and updated CT study worksheet to the TCEQ for review and approval. *An update to the previous sedimentation basin tracer study is not anticipated to be needed and is not included in the scope of services.
