AMENDMENT NO. 1 TO
CITY OF FORT WORTH CONTRACT 54427
This Amendment is entered into by and between the City of Fort Worth (hereafter
"City"), a home rule municipality, with its principal place of business at 200 Texas Street, Fort
Worth, Texas, and Elise Amara (hereafter "Licensor"). City and Licensor shall be referred to
individually as a"Party" and collectively as the "Parties."
WHEREAS,the Parties have previously entered into City of Fort Worth City Secretary
Contract No. 54427 (the "Agreement"); and
WHEREAS,the Agreement was executed for Licensor to grant the City a non-exclusive,
royalty-free license to use and make available to Library patrons certain musical compositions,
files, recordings or other creative works through permanent download and streaming via a web-
based content sharing device from the Library's website ("Agreement"); and
WHEREAS, the Agreement began on September 4, 2020 ("Effective Date") for a three
year term("Initial Term"); and
WHEREAS, the Parties acknowledge that there was a typographical error in which
Licensor's name was inadvertently misspelled on both the first page and the Signature page of the
Agreement as "Shaya Elise Fuller"; and
WHEREAS, the Parties acknowledge that the correct spelling of Licensor's name is
"Shayna Elise Fuller"; and
WHEREAS, the Parties now wish to amend the Agreement to allow for the correct
spelling of Licensor's name on both the first page and the Signature page of the Agreement;
WHEREAS, Parties agree no other changes to the Agreement are being requested and
all other terms not amended herein shall remain in full force and effect; and
NOW, THEREFORE, the Parties, acting herein by the through their duly authorized
representatives, agree to the following terms, which amend the Agreement as follows:

AMENDMENTS TO AGREEMENT
The following terms are hereby amended to replace the referenced terms in the Agreement
and shall be binding and enforceable as if they were originally included therein.
A. "Name of individual authorized to sign on behalf of below-named artist(s) or musical
group:", located on page 1 of the Agreement below the heading, is hereby amended
and replaced in its entirety with the following:

"Name of individual authorized to sign on behalf of below-named artist(s) or musical
group:" Shayna Elise Fuller.
B. LICENSOR: Name, located on page 7 of the Agreement below Licensor's signature,
is hereby amended and replaced in its entirety with the following:
Name: Shayna Elise Fuller

ALL OTHER TERMS SHALL REMAIN THE SAME
All other provisions of the Agreement which are not expressly amended herein shall
remain in full force and effect.
3.
ELECTRONIC SIGNATURE
This Amendment may be executed in multiple counterparts, each of which shall be
an original and all of which shall constitute one and the same instrument.A facsimile copy
or computer image, such as a PDF or tiff image, or a signature, shall be treated as and shall
have the same effect as anoriginal.
