Appearance Release
The City of Fort Worth Texas Public Library the S onsor is producing an online
program for youth entitled "Learn Dream Do: Sky Full of Stories" (the Program),
and has requested me to record a video that details my personal experiences
as a NASA astronaut board-certified in Internal and Aerospace Medicine (my
Performance) for the Program. I have so agreed.
I have no objection to the reproduction, use, exhibition, display, broadcast,
webcast, and distribution by the Sponsor and those acting on its behalf, in any
media of the video of my Performance for the Program, in perpetuity and
throughout the world, nor to the use of my name, photograph, likeness, voice,
and biographical or other NASA audiovisual material to the extent used in
conjunction therewith, nor for the incidental publicity or promotion thereof,
provided that none of the above be used in any manner such as to constitute or
otherwise imply an endorsement of any entity, product, or service by myself or
by the National Aeronautics and Space Administration (NASA).
My Performance for the Program is considered to be part of my official duties as
an employee of the United States Government. Government Standards of
Ethical Conduct preclude me from receiving any monetary consideration for my
participation.
