EXTENSION OF VEHICLE USE AGREEMENT
The National Insurance Crime Bureau ("NICB") and the City of Fort Worth ("Fort
Worth") entered into the following Vehicle Usage Agreements:
1. In 2007 (Agreement dated December 4, 2007) ("Original Agreement'),
2. A second Agreement in 2009 (Fort Worth City Secretary Contract
No. 39381 dated October 29, 2009) ("Second Agreement"), and
3. A third Agreement (Fort Worth City Secretary Contract No. 39699
dated January 22, 2010) ("Third Agreement")
in which NICB leased to Fort Worth the vehicle listed below for the purpose of
investigation of insurance-related crime and/or fraud.

WHEREAS, the terms of the Original, Second and Third Agreements were one
year each, effective the date the vehicle was delivered into the custody of the Fort Worth
Police Department.
WHEREAS, NICB and Fort Worth wish to renew the Third Agreement for an
additional one (1) year/two (2) period from i!- %- gloio to P - 3` -
NOW THEREFORE, in consideration of the mutual agreements set forth in the
Third Agreement, other good and valuable consideration, and the conditions and
agreements set forth below, the parties agree as follows:
1. The parties agree that the Third Agreement shall be renewed for a one
one (1) year/twe (2) period renewal period from _ V -`7 -
/1- cY-19 11.
2. All other terms and conditions of the Third Agreement, as amended,
remain unchanged and in full force and effect.
