In accordance with the Professional Services Agreement signed between Sentari
Technologies, Inc. ("Contractor") and the City of Fort Worth ("Client" or "City") effective
as of November 6, 2013 and approved by M&C C-26551 on November 5, 2013 (the
"Agreement"), this Appendix B-28B is incorporated into the Agreement and hereby
reads as follows:
1. The term of this work order shall be extended to end when all services have
been completed under this work order as contemplated herein, but not later
than the end date set forth below.
2. This Work Order incorporates by reference all of the terms and conditions of
the Professional Services Agreement, specifically the services to be provided
under Section 1 of said agreement.
3. The services to be performed under this Work Order shall be as follows:
IT DESKTOP OFFICE ADMINISTRATIVE ASSISTANT
a) Evaluate, monitor, schedule and coordinate computer installations, moves,
and other activities for City Departments;
b) Research and validate IT Desktop assets for replacement;
c) Dispatch IT Desktop asset installation or move work orders using ITSM
work order management system;
d) Process IT Desktop asset warranty repair work;
e) Purchase parts for IT Desktop assets;
f) Produce monthly IT Desktop activity reports;
g) Compile and prepare fiscal, statistical and related program reports;
h) Calculate financial data, update accounts and provide program
expenditures and available balances;
i) Maintain files and update/input records;
j) Maintain records system, review application and data accuracy and
content;
k) Perform related duties and responsibilities as required.

Payment Rate: $35.00 per hour
5. This Appendix B-28B shall terminate immediately in the event the contract
personnel does not begin to provide contract services to the City under this
Appendix as of the Start Date stated above. If the contract personnel begins
to provide services, but Contractor provides notice to the City that the contract
personnel will no longer provide services under the Appendix, and such
notice is effective prior to the End Date stated above, then this Appendix shall
terminate immediately as of the effective date of the notice from Contractor.
