Total: 100 passes granted for FY 2013w2014
By signing this agreement, I certify that I am an authorized representative of the agency
n amed above. I hereby certify that the passes obtained by our agency through this
program will not be sold for any amount to any person. The passes will be given away
u nder the guidelines established by our agency to persons determined to be in need of
public transportation that are not able to pay the fare to ride public transportation. I
u nderstand that if any passes are sold, our agency will be disqualified from the program.
The agency is responsible for the passes granted. The T will not replace lost, stolen or
damaged passes.
A report must be filled out when picking up the next allotment of passes.
Please inform the receptionist if you are not able to make your appointment; failure to
do so will result in loss of passes for that allotment.
By signing this agreement, I certify that the agency will follow the rules of the program
as outlined above.
