TO CITY SECRETARY CONTRACT
WHEREAS, the City of Fort Worth (CITY) and Criado &
Associates, Inc., (ENGINEER) made and entered into City
Secretary Contract No. 43534, (the CONTRACT) which was
authorized by M&C C-25758 on the 24th day of July, 2012 in the
amount of $184,347.00; and
WHEREAS, the CONTRACT involves engineering services for
the following project:
Water and Sanitary Sewer Replacement Contract 2011,
WSM-A.
WHEREAS, it has become necessary to execute Amendment
No.1 to the CONTRACT to include an increased scope of work and
revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and
through their duly authorized representatives, enter into the
following agreement, which amends the CONTRACT:
1.
Article I of the CONTRACT is amended to include the
additional engineering services specified in a proposal dated
August 30, 2013, a copy of which is attached hereto and
incorporated herein. The cost to City for the additional design
services to be performed by Engineer totals $47,460.00.
2.
Article II of the CONTRACT is amended to provide for an
increase in the fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall be
an amount of $231,807.00.
3.
All other provisions of the Contract, which are not
expressly amended herein, shall remain in full force and
effect. ;

CRIADO A Assoctatea, Inc. (CRlADo) is pleased to present this Amendment No.1 to the Engineering Services
Contract for City Secretary Contract No. 01928 "Water and Sanitary Sewer Replacement Contract 2011, WSM-A".
This amendment includes four separate parts, as described below:
SURVEYING SERVICES FOR ALL PHASES OF DE31GN
cR1At74 will perform land surveying services as required to collect horizontal and vertical elevations and other
information needed in design and preparation of plans for the project. Information gathered during the survey
shall include topographic data, elevations of all sanitary and adjacent storm sewers, rim/Invert elevations, location
of buried utilities, structures, trees (measure caliper, identify overall canopy, and have qualified arbodst identity
species of trees), and otherfeatures relevant to the final plan sheets. Existing drainage at intersections will be
verified by field surveys, Spot elevations will be shown on Intersection layouts with cross slope to fit intersecting
grade lines. The estimated effort required to complete the Design Survey is $1703209
ENGINEERING SERVICES FOR PRELIMINARY DESIGN (60-PERCE11411]
CRIAno will prepare Preliminary Design Drawings and Project Specifications & Est(mates (P5&E) for the
proposeA lateral improvements of this amendment. Plan and profile geometry will be coordinated with the
proposed and existing sanitary sewer laterals using a 1" = 20' horizontal and 1" = 4' vertical scale, The design
drawings will also include a Cover Sheet, Overall Project Layout, Survey Control, Traffic Control Plans, and a
Storm Water Pollution Prevention Plan (SW3P). The estimated effort required to complete the Preliminary Design
is $27,126.
ENGINEERING SERVICES FOR FINAL DESIGN (90J100-PERCENT]
CRIADO wil! prepare Draft Fnal Plans fora 90-percent construction plan review meeting with the CITY, foilawed
by the submission of the Construction Documents (100-percent) to the CITY per the approved Project Schedule.
CKADO will also provide a final design estimate of probable construction cast with both the 90-percent and 100-
percent design packages using ONLY standard CITY bid items. The estimated effort required to complete the
Final Design is $3,014.
