FIRST AMENDMENT AND FIRST EXTENSION TO CITY OF FORT WORTH
CONTRACT 54871 BETWEEN THE CITY OF FORT WORTH AND RONALD
MCDONALD HOUSE OF FORT WORTH
This Amendment is made and entered into by and between the City of Fort Worth, a home-
rule municipality of the State of Texas (hereinafter referred to as the "City") and Ronald
McDonald House of Fort Worth (hereinafter referred to as the "Agency"), a Texas non-profit
corporation. City and Agency may be individually referred to herein as"Party"and collectively as
"Parties."
RECITALS
WHEREAS, on November 18, 2020, the City entered into an Agreement with Agency to
provide services more specifically described in the agreement, City Secretary Office (CSO)
Contract No. 54871, (the "Agreement"); and
WHEREAS, the Parties wish to amend the Agreement to appropriate additional funds to
Agency for expenses related to COVID-19 equipment. Further,the Parties wish to extend the term
of the Agreement, as contemplated by the terms of the Agreement.
NOW, THEREFORE, the Parties, acting herein by the through their duly authorized
representatives, enter into the following agreement:
I.
AMENDMENTS
Pursuant to the terms of the Agreement, the Contract Term is hereby amended and extended with
a new expiration date of January 31, 2021.
EXHIBIT "B" Budget attached to the Agreement is hereby amended and replaced in its entirety
with the Exhibit B attached hereto:

ALL OTHER TERMS SHALL REMAIN THE SAME
All other provisions of the Agreement which are not expressly amended herein are not
affected by this amendment.

ELECTRONIC SIGNATURE
This Amendment may be executed in multiple counterparts, each of which shall be
an original and all of which shall constitute one and the same instrument.A facsimile copy
or computer image, such as a PDF or tiff image, or a signature, shall be treated as and shall
have the same effect as anoriginal.
