Addendum No. 1
Statement of Work
This Addendum No. 1 - Statement of Work (SOW) is made as of this 29th day of January,
2021, under the terms and conditions established in the PROFESSIONAL SERVICES
AGREEMENT between the City of Fort Worth ("City") and Strategic Government
Resources, Inc. ("Consultant"), the same being Fort Worth City Secretary Contract (CSC)
No. 54728 (the "Agreement" or"PSA").
This Statement of Work is made for the following purposes, consistent with the services
defined in the Agreement: see attached Scope of Work provided by Consultant, dated
January 22, 2021.
Compensation, Reimbursable Expenses, and Billing Terms: Processing and payment of
Fees and Reimbursable Expenses, including the structure of payments and invoicing
therefor, shall be in accordance with the Agreement.
Consultant shall submit invoices for Fees and Reimbursable Expenses to the
Talent Acquisition Manager and OD Admin Assistant within the Human Resources
Department at City of Fort Worth, 200 Texas Street, Fort Worth, Texas 76102.

Scope of Work Agreement between the City of Fort Worth and SGR for Creating
Discussion Guides for Selected Books for Book Clubs
January 22, 2021
1. SGR will create six "Discussion Guides" for each of the following two books:
• Blind Spot by Mahzarin R Banaji/Anthony G Greenwald
• Mastering Civility by Christine Porath
2. These "Discussion Guides" will follow the format given to us from the City of Fort
Worth and which was followed by SGR in creating the previous discussion
guides that were used for the Book Clubs.
3. Each of the Discussion Guides will cover roughly the same amount of reading so
that the book club can systematically read through the book in a timely manner.
4. These Discussion Guides will be sent to the City of Fort Worth electronically to
be used by the facilitators leading the discussions.
5. The cost will be $1,800 per book for a total of$3,600.00
6. SGR will invoice the City of Fort Worth after the Discussion Guides have been
completed and sent to the City.
If this agreement accurately reflects your expectations and understanding of our agreement, please
execute two copies and return one to me for my files.
