Contract No. CSC No. 53045 (the "Contract")
Renewal Term No. 2: January 1. 2021 to December 31, 2021
The above referenced Contract with the City of Fort Worth expired on December 31, 2019
(the "Expiration Date"). The agreement also allows for up five renewal terms of one-year each.
Pursuant to the Contract, contract renewals are at the sole option of the City. It was the intention
of City to renew the agreement for its first renewal term of January 1, 2020 to December 31,2020.
And, in fact, City and Vendor did perform under the same terms and conditions of the original
term of the agreement for a first renewal term of January 1, 2020 through December 31, 2020.
This letter is to inform you that the City is exercising its right to renew the Contract for its second
renewal term to continue the agreement for an additional one (1) year period, which will begin
immediately after the Expiration Date. The second renewal term, as indicated above, will be
January 1, 2021 to December 31, 2021. All other terms and conditions of the Contract remain
unchanged. Please return this signed acknowledgement letter, along with a copy of your
current insurance certificate, to the address set forth below, acknowledging receipt of the
Contract Renewal Notice. Failure to provide a signed acknowledgment does not affect the
renewal.

RE: City Secretary Contract No. 53045; Renewal No. 1.
The City of Fort Worth executed CSC No. 53045 with ANSI National Accreditation Board,
LLC.The agreement allows for up to five renewal terms of one-year each.Pursuant to the Contract,
contract renewals are at the sole option of the City. It was the intention of City to renew the
agreement for its first renewal term of January 1, 2020 to December 31, 2020. And, in fact, City
and Vendor did perform under the same terms and conditions of the original term of the agreement
for a first renewal term of January 1, 2020 through December 31, 2020. The purpose of this letter
is to memorialize the fact that the parties, by their conduct, renewed the agreement for its first
renewal term.
