FIRST EXTENSION OF CITY SECRETARY
CONTRACT NO 38212
THIS FIRST EXTENSION to City Secretary Contract No. 38212 (hereinafter "Extension") is
entered into by and between the City of Fort Worth("City"),a home-rule municipality in the State of
Texas, acting through Fernando Costa, its duly authorized Assistant City Man�gger, and CRT
Processing,LLC d/b/a Environmental Light Recyclers,by and through DIANE /N. IMN UIc" ,its
duly authorized pro ' ' f ~ "�� ' *, hereinafter called "Contractor".
WHEREAS,the City and Contractor entered into that certain agreement identified as City Secretary
Contract No. 38212(also the"Contract")commencing on December 1,2008 and ending November
30, 2009 with two consecutive one year renewal terms for packing, transportation and disposal of
various lamp,ballast and electronic waste; and
WHEREAS,the City and Contractor wish to exercise the first extension option;
WHEREAS,the parties agree that all other conditions, terms and pricing shall remain the same as
stated in City Secretary Contract No. 38212.
NOW,THEREFORE,for and in consideration of the mutual covenants contained herein,and other
good and valuable consideration,the receipt and sufficiency of which are hereby acknowledged and
confessed, the parties hereto agree as follows:
The Contract with CRT Processing, LLC d/b/a Environmental Light Recyclers, for packing,
transportation and disposal of various lamp, ballast and electronic waste known as City Secretary
Contract No. 38212 shall be extended pursuant to Article 1 "Term"for its first one(1)year renewal
beginning December 1, 2009 and ending November 30, 2010.
