Re: City of Fort Worth Contract Extension Notice
CSC# No. 48773 (the "Contract") Extension
The above referenced contract with the City of Fort Worth expires on March 3, 2021 (the
"expiration date"). This letter is to inform you the City is requesting to extend the Contract # 48773
for an additional eighty-seven (87) days, which will begin immediately after the Expiration Date,
until June 1, 2021. All other terms and conditions of the Contract remain unchanged.
To agree to renew the Contract for an additional eighty-seven (87) day period, please
electronically sign and return this request to exercise renewal option. Also please send a
copy of your current insurance certificate to Mr. Roger Grantham, City of Fort Worth
Environmental Supervisor at the following email address:
