Statement of Work
This Addendum No. 4- Statement of Wark (SOW) is made as of this 25th day of March, 2021,
under ilae terms and conditians established in the MASTER PROFESSIONAL SERVICES
AGREEMENT between the City of Fort Worth ("Cit�') and CornerStone Staffing ("Vendor"},
the saine being Fort Worth City Secretary Contract (CSC) No. 54720 (the "Master
Agreeix�ent" or "MSA").
This Statement of Work is made for the followingpurposes, consistent with the services de�ned in the
Master Agreenaent: Providing temporary staffing services to the City af Fort Worth for the positi�ons
listed belovw. In particular, Vendor shall perform those functions identifed below in the Scope af
Services, as such ser�ices are rnore fially detailed in t�e MSA.
Section A. Scope of �ervices & Sclnedule
Scope of Service: The Nei�hbarhood Services Department (NSD} is requestin� 3 temporary staff #o
sezve as Eli i�bility Specialist. NSD receives numerous State and �'ederal �rants used to provide
,financial assistance to persons that are low-income, experienced recent crisis, had a loss of income
due to COVID and/or seeking job training/certification.to earn a livable wage. Wrth the increase in
fundin� and pro�rams, there has been an increase in request for assistance. All a�p�ications received
must be screened for required documents before assi�nin� to be processed. The increased volume
requires additionaI staff to review all a�lications ta assist i�n recei�i� t�y notif cation re a� rding
the status of their application, pro ra� m eli ig bility and receive requested assistance.

Section C. Compensation, Reimbursable Expenses, and Billing Terms: Processing and
paytnent of fees, including the structure ofpayments and invoicing ihereof, shalI be in accordance with
the Master Professional Services Agreement.
The total fee that shall be paid by City to Vendor for these services shall be in accordance with the
Master Service Agreement and, specifically, E�hibit "B"� Price SchedUle. �in subzxiitting invoices,
Vendor shall reference the appropriate �tatement of Work. Further, the Vendar shall separately lisi
the total amount for fees in which invoices were previously subtnitted during the then�cUrreni annual
contract period (either initial term or rene�ral period) and the totaI amount of fees for the eniire
perioci, including those amounts beizag requested under the then-current invoice.

Vendor shall subrnit invoices for Fees to Vella Edwards, within the Nei�hborhood Services
Department and also submit a co�y to the Talent Acquisition Manager in the Human
Resources Department, 20Q Texas Street, Fort Worth, Texas 76142. `
