"This Addendum No. 9- Statement of Work (SQW} is made as af this 18th day of May, 2021,
under the t�rms and conditions established in the MASTER PRflFESSI�NAL SERVICES
AGREEMENT between the City ❑f Fort Warth ("City"} ai�d Carnerstanc ("Vendor"}, the same
being Fort Wprth City Secretary Cantract (CSC} Na. 5472d (the "MasCer Agreeinent" or "MSA"}.
This Statement of Work is ma�ie fQr th� follawing purpases, �onsistent with the services det7ned
in tf�e Master Agre�ment: Pra�idi3�g temporary staff'ing ser��ices to tihe City of Fort Warth for t�e
�asitions listed 6elow. In particular, Vendor shall perform thtise functions ic�Entified beEaw in ti�e
Scope of Servi�es, as such ser�ices are more fully detailed in the MSA.

The TPWIStreets and Stormwater Operations Divisian is requesting ta extend the temporary
assignment for Sade Rasach. Sad� woulc! continue to pro�ide assistance with payroll, HR
furtctions, the procurement pr�cess and various administrati�e projects witliin the di�ision. 'I'he
num6er of ho�srs fhis emplayee wauld continue to wark is 30 hours per week.

with the Master Professional Ser�ices A�reement.
The tatal fee that shall be paid hy City to Ver�d[�r for these services shal] be in accordance with the
Master Ser�ice Agreement and, specifi�a[ly, Exhibii "S"- Price Schedule. In submittin� in�oices,
Vcndar shalf reference the appropriate Statemer�t of Wark. Fur�her, the V�ndar shall separately
list the totai ainaunt far fees in whi�h invoices were pre�iousty submitted during the then-Current
annual contract period (eith�r initial term ar renewal period} and the total amount af fees for the
entire period, including ihose amouilts being requested under the t11ei�-current in�oi�e.
Vendor shall submit in�oices for Fees to Traci.Akin r�fartworthtexas. o� within
Trans nrtation and Puhlic Works and also submit a eopy to the Talent Acquisitinn Manager
in the Human Resaurces Dspartment, ZDd Texas Street, Fort Warth, Texas 761�2.
