This Amenc�ment of Port Warth City Secretary Contract No. 5472� {"Agreenneni"} is
made asid entered into 6y and betw�en ihe City af Fart Worth, a hoine rudc municipal corporation
❑f the State of Texas ("�ity"), acting herein by and thraugh ]esus Chapa, its duly authorized
I]eputy City Manager, and Smith Tempr�raries, Inc. dba CarnerStone Stat'fing �"Vendor"},
acting 6y and throu�ll Colby Wal�rop, its dUly authori7ed Setiiar Bran�h Manager. City and
Vendor shall be referred to individualIy as a"Party" and collectively as the "Parties."
WHEREAS, the f'arties have previous�y entered into City of Fort Vl�arth City Secretary
Contract No. 54720 (the ``Agreement"); and
WHEREAS, the �io�i-ex�[usi�e Agreemeni was executed far Vendor to pro�ide City
with temporary staffing ser�ices, including professionals, clericai workers and ma�ival labar
("Temporary Warkers"}, on an as nee�ed Iaasis; and
WHEREAS, pUrsuant to 5ectiar� 3,2 of the Master Agreemei�t, when services are needed,
the Parties are required ta execu[e a StateEnent ❑f Work ("SQ W") to list the sery ices requested by
City as well as the prajected time frame and amoU�its fnr said requested ser�ices; and
WHEREAS, the Parties pre�iausly en#ered into a 5�W for Vendar to pra�ide City with
temporary stafFing ser�vices for the pasition tit[ed "Office Assistant" for the Neighhorhood Ser�ices
Department (CSC No. 54720-A�J7}; and
WHEI2EAS, the Parties agree to terminate the request for ser�ices under CSC No. 5472�-
AD7 and hereby agree that r�a work wi�l be campleted or com�e�lsated under C5C Na. 5472a-
AD7.
N�W, THEREF�RE, the Parti�s, acting herein by the througll their duly auth�rized
representati�es, agree to tlie foll�wing terms, which amend the Agreement as fallaws:
Ti�e parties agree that the recitals set forth abo�e are true and cnrre�t and are hereby incorporated
into thc Agreement, as set foi�h in fu[1.

CSC Na. 5472Q-AI]7 {"Addendusn No. 71Statement oiWork"} afthe Agreement, is hereby
defetecf in its entirety. No ��ork wiIl be completed or compensated under CSC No. 54720-AD7.

A1� other pro�asions of the Agreement whic� are not expressly amended herein shall
remain in full farce and effect.
