AMENDMENT NO. 3 TO
CITY SECRETARY CONTRACT NO. 25503
INVENTORY AUDIT AND RECONCILIATION AGREEMENT
WITH
CAROLYN ARNOLD COMMUNICATIONS CONSULTANT, INC.
This Amendment (hereafter "Amendment") is entered into by and between the City of
Fort Worth (hereafter "City" or "Customer") and Carolyn Amold Communications Consultant,
Inc., (hereafter "Consultant" or "CACC"), individually referred to as a "party" and collectively
referred to as the "parties".
WHEREAS, on or about November 19, 1999, the parties made and entered into an
Inventory Audit and Reconciliation Agreement under the terms and conditions set forth in the
Agreement. The Agreement is a public document on file in the City's Secretary Office and
referenced as City Secretary Contract No. 25503, and
WHEREAS, on or about November 4, 2011, the parties subsequently amended the
Agreement via City Secretary Contract No. 25503-Al to extend the term and modify other
matters, and further amended the Agreement on April 16, 2012 to extend the term for one
month. City Secretary Contract Nos. 25503; 25503-A'l and 25503-A2 are hereby collectively
referred to as the "Agreement"; and
WHEREAS, the Agreement involves audit and reconciliation of services and billing for
telephone utilities, and
WHEREAS, the Agreement expired on April 31, 2012; and
WHEREAS, the parties Wish to amend the Agreement to extend the term for thirty days.
NOW THEREFORE, the City and CACC, acting herein by and through their duly
authorized representatives, enter into the following agreement.

Section 3, Term, shall be modified to extend the terra of the Agreement. The extended
term shall commence on November 1, 2012 and expire on November 30, 2012. No further
extensions shall be granted under this Agreement, unless otherwise approved in writing by the
parties.
2.
All other provisions of the Agreement that are not expressly amended herein shall
remain in full force and effect.
