Original Contract Amount: $80,000
Amended Contract Amount: $100,000
The above referenced contract with the City of Fort Worth, as amended, automatically
renews on October 1 of each year,with up to 4 renewals,for a total of 5 years. On October 1,2020,
the contract renewed automatically and has been in effect; however no formal renewal notice was
sent. This letter is to update the record as to the first renewal and to inform you that the City is
exercising its right to renew the contract for the second time in an amount up to $100,000, which
will be effective October 1, 2021 upon execution by the designated Assistant City Manager. All
other terms and conditions of the contract,as amended,remain unchanged. Please sign and return
the second page of this letter, along with a copy of your current insurance certificate, to the
undersigned.
