FIRST AMENDMENT TO CITY SECRETARY CONTRACT NO. 55483
WHEREAS the City of Fort Worth ("City") and Volunteers of America("Agency") made
and entered into City Secretary Contract No. 55483 ("Contract");
WHEREAS, City receives grant monies from the United States Department of Housing
and Urban Development (HUD) through the Community Development Block Grant ("CDBG
CV3") Program, Program No. B-20-MW-48-0010, Catalog of Federal Domestic Assistance No.
14.218;
WHEREAS, City has received an additional allocation of CDBG grant monies under the
Coronavirus Aid, Relief, and Economic Security Act ("CARES" Act), referenced by HUD as
"CDBGCV3"
WHEREAS, the Agency has requested to change the use of the CDBG-CV3 funds for an
emergency rental assistance program to serve individuals and families, 100% of whom must be
low and moderate income;.
WHEREAS,the Agency request to amend the Program Summary to include low-moderate
income families in addition to homeless individuals;
WHEREAS, City believes that the requested amendment will help Agency meet the
program's goals and further the interests of both Agency and City; and
WHEREAS,it is the mutual desire of City and Agency to amend the Contract to complete
the Program and meet Contract objectives.
NOW, THEREFORE, City and Agency hereby agree to amend the Contract as follows:
I.
Exhibit"A"Program Summary and detail attached to the Contract is hereby amended and
replaced in its entirety with EXHIBIT "A" and detail,revised 9/28/2021, attached hereto.
II.
Unless otherwise specified, this amendment is effective as of the Effective Date of this
Amendment.

III.
All terms and conditions of the Contract not amended herein remain unaffected and in
full force and effect, are binding on the Parties and are hereby ratified by the Parties. Capitalized
terms not defined herein shall have the meanings assigned to them in the Contract.

VOATX CARES- City of Fort Worth Program Summary

Capitalized terms not defined herein shall have meanings assigned them in the contract.
PROGRAM:
The Program will provide residents Short-Term Rental and Mortgage Assistance and Homeless
Prevention for those Fort Worth households affected by COVID-19. The program will provide
payments rent or mortgage assistance for up to six(6)months with a maximum of 60 days in arrears.
The program is for households who have incomes at or below 80% of the area median income.
Participants will be required to provide documentation of the impact of COVID on their household and
pay no more than 30% of their income toward their rental cost. The Program will provide services to
CDBG-CV eligible client residing in the City of Fort Worth.
CDBG-CV3 funds will pay for direct rental assistance and mortgage assistance for clients; Costs of
staff to administer the program(case managers,intake specialists,etc.),and other related costs such as
general liability insurance,accounting services and other payroll costs.

The Program will provide services to CDBG-CV eligible client residing in the City of Fort Worth.
Agency shall not change any locations without prior City approval. Costs incurred at any locations that
have not been approved by City shall not be reimbursed.
