Services to be, provided also include some bidding and construction services, environmental permitting, hydrologic and hydraulic modeling, and right-of-way/easement assistance. The project, is separated in to two phases; Phase 1 consists of a multi-use storm water detention basin and associated storm drain system improvements located generally in the southern and eastern part of the watershed, and Phase, 2 (previously known as Phase 2 and 3) consisting of storm drain system improvements throughout the remainder of the watershed. Phase I was designed in 2010-2011 and is being constructed in 2011-2012. Phase 2 is scheduled' to be designed in 2011-2012 and construction will begin in late 20,13. At time of this amendment, the 30% design for Phase, 2, has been completed and reviewed by the City, and much of the phase has been taken to the 60% level of design. I) ery m This amendment is organized into two parts. ( Phase 2 design s ices odifications due to a realignment of the project and change of the type of facilities beli'ng designed'- and (II) As-needed services to support several potential permitting and construction administration needs that have or may arise as the work progresses. Part I is provided on a lump sum basis, sim:ilar to the original design contract, Part II is provided on a time and materials basis to reflect the uncertain nature of the work. BASIC SCOPE Off` SERVICES 1. Shgdow Creek Box Cullivert Reafignment&Channel Imigrovements Background During the process of performing the, Phase 2 design, work, a workshop was held with City staff to consider several alignment alternatives for the outfall location of a large box culvert. 'The preferred alternative entails re-routing the box culvert to an existing channel located south of Shadow Creek Court, the existing trunk line system currently outfall's here via a reinforced concrete arch culvert under Weiler Boulevard, that has been shown to be significantly undersized, so re-routing the diversion box culvert to this same location will require significant improvements to the receiving channel in order to U�pidate,d: October 30, 2012 Page 1 Printed: 1/16/2013 1:58 PIVI ............. prevent erosion and flooding of private property. The area in question can he seen in Attachment A-1. The reason, this alignment was preferred from the design workshop is that it reduces the construction in Weiler and avoids. existing utility relocations, traffic control, and general public interference (existing utilities were not available during the planning and 30 percent design phases). The selected alternative, also utilizes,the existing channel 'into, the golf course, whereas the studied alignment,would be close to an existing golf course hole, and would likely change the play of the hole both during construction and long term. All things considered,, the change of alignment was seen as a cost savings to the project and worthy of the design change. This amendment therefore contains the estimated effort required to incorporate these channel improvements, and it also details substitutions of existing scope items with new items related to this design change and other changes, described in detail below. This design will include the location, alignment/ limits,, and top-of-wall elevations for the proposed MagnumStone modular block retaiining, walls that will line both sides of the outfall, channel. 9 ii. Jacobs will provide design of energy dissipators, and, an, approximately 35" long retaining wall with pipe penetrations (headwall), including all necessalry calculations, drawings, details, and quality reviews. Jacobs will provide construction administration (CA) review of retaining wall submittals (concrete/relbar) by the City's contractor. The prlolpolsed Magnu�mStone modular block retaining walls that will line the channel downstream of the h,ea,dwall will be a "des,ignated design," meaning that structural design calculations will be provided by the contractor and must, be signed and, sealed by a registered professional en�gineer. Jacobs will provide CA review of this modular block wall and foundation design submittal. (See Phase II CA services) 1 111 Survey and S,ubsu�rface Utility E,ng neerincl (SU,E). See attached scope from ANA, included and made a part of this contract as Attachment A-2. Preparation of any temporary and permanent easements will be performed per the original contract scope,., iv. Environmental Permitting: Jacobs will expand the scope of the USACE environmental permit investigation and, preparation to include modifications to the existing channel'. The environmental services to be performed for the stream blank modification associated with Phase H will:1 include 'the performancel of a preliminary Jurisdictional delineation and corresponding report identifying the Section 404 jurisdictional' limits of the stream and other associated areas subject to Section �404 that may, be associated with the stream,. I vi., Hvdraulic Model i'ng-,,(H EC-RAS and In�foWor ', Jacobs will perform preliminary sizing of the channel using HEC-RAS, using flows taken from the InfoWorks SD model prepared under Amendment #1, and this HEC-�RAS model will be used to establish proposed grading and other design elements for the 30% submittal. After approval' of the 30% design, any changes required for the 601% submittal will be modeled using the HEC-RAS model; subsequently,, the proposed channel grading will then be added to the InfoWorks SD model and the reSL11tS of both models compared for reasonableness; this, comparison is necessary since the InfoWorks SD channell modeling capabilities are not as well- understood or widely accepted as HEC-RAS. It is not expected that significant changes will be requ,ired going from 6101%, to fina�l drawings (90% and 100%), but any changes, that are made to the design will be incorporated into the models for final SLibmittal of same. If significant changes are required, additional scope and budget may be necessary. I Proposed oioe from J,,a,c,quelin,e to Yolanda: The original concept plan for this improvement alignment, as determined during the watershed study phase of the project, was to, design and install a new relief pipe running generally, parallel to the existing natural drainage channel, with the constraint of keeping the pipe ,within the existing easement (most of the channel does not lie within the easement). After obtaining survey information and making another site visit with City staff, it was determined that this proposed alignment along the existing easement would r�e�qlu,i�re deep trench ing in some locations due to a steep, hillslope that exists, in some of the backyards. This trenching would be very disruptive to established backyards, and Could require the City to: remove and replace landscaping, vegetal!tion, fences, decks and footbridges,that span the drainage, channe�l, a gazebo,, utility poles, outbuildings, and other similar backyard items. Based on preliminary model results simulating, the new proploiseld 'i'�r-riprolv,emie�n!ts, describled in scope item numbers, 4 a�nd 5 below, it, appears that storm water flooding can be reduced in this backyard channel without installation of this �pipe. The previous scope envisioned improved inlets at the s,ump on Jacqueline near the upstream end of this channel; this change would eliminate those new inlets as well. 4. Inlets near Jacqueline olanda 'intersection: A pa:ir of proposed inlets on either side I of a valley gutter on J�acque�linle, near Weiler,, are,, currently planned to be piped east (down Jacqueline) to the proposed pipe (parallel to the existing channel, eliminated In order to provide in item #3 above) that runs behind the houses along Yolla:nda relief to the, existing system, at the sump in Jacqueline and to the existing natural drainage channel, it was decided to instead take the flow from these inlets (approximately 3:0 cfs) and carry it west to Weiler, extending the proposed system farther to the north along Weiler to connect. Survey and SUE was not obtained for part of this area in the original scope, and Jacobs does not propose to collect it at this time. Jacobs, proposes, to digitize surface features to fill in the gap between the existing survey for Weiler and Jacqueline; the only utility potentially im,pacted Updated: October 30, 2012 Page 5 Printed: 1/16/20113 1#058 PM appears to be a 12-inch water line, and missing information for thi's can be filled in from plans. 5. U rode Yolanda c"r"ossina: The existing 2-36"' reinforced concrete pipe structure under Yolanda (between Jacquel�ine and Danciger) appears to be undersized based on preliminary model results: 'improvements, here were not part of the concept flan developed during the watershed study phase, but current modell�ing and field assessment suggests a new structure, may be required. This scope change would upgrade this crossing to a larger box culvert that would reduce overtopping potential and 'improve capacity in the natural drainage channel upstream. 6. Parks conversion Dermi it: The existing scope for parks conversion permit is moved from the lump sum portion of the budget to the as-neleded�, time and materials section (Part II) since the need for this permit is uncertain at this time and will be determined as the design progresses. 7. SWPPP change to iSWM Checklist.- This provides for the, changefrom to the ci standard olw being used,. the iSWM Site Plan Checklist, instead of the consultant- supplied SWRPP rocess be,in,g used at the beiciinningof,the projec . B. Defiverables I. Conce tual drawinas. 30% design package submittal showing the revised alignments, dimensions, and other key design elements, for the items revised by this scope 1-5),, including how they tie into, the rest o!f the project-- 2. 30% Review meeting., Jacobs, will prepare for and attend one (1) project review meeting with City staff to, present, discuss, and gather feedback from the 30% submittal for the items revised by this scope (1-5). 3. 601% Dira i in Jacobs will incorporate the 30% design review comments for items, revised by this scope (1 5) into the 60% dl:es,igin package and submit to the City, mal ingi one submittal for alli of Phase 2. The Phase 2 60% design review meeting is covered by the original scope, 4. Final-(910% and 100%) Dra.w,i.,n., Jacobs will incorporate the 160% design review comments for items revised by this scope (1-51) into the 90% design package and submit to the City, making one submittal for all of Phase 2. The Phase 2 90% design review meeting is covered by the original scope. 5. Revised I I _geoltechni'cal re Jacobs will provide a revised geotechnical report that includes, previous bores, analysis, and recommendations, as well as information for new bores acquired for the new alignment and channel improvements. This report will include global stability ana,l�ysis, for the proposed: retaining walls lining the channel. These walls will be a designated design, and this analysis, will be provided to the contractor for that purpose. Jacobs structural engineers, will review and approve the submitted design as appropriate. C. As,sumptions 1. Hydrology (S,CS method) developed during the watershed planning study and refined during the original scope and previous amendments to this contract will be used as a Updated: October 30, 2012 Page 6 Printed: 1/16/2013 1-058 PM starting point for this modeling effort and refined as required by the design to accommodate new proposed inlets. 2. The new proposed channel will be designed to carry the 100-year storm. I This scope assumes that no street grades will change due to any proposed imp rove run ents. 4. Should the USACE, re quire, comperisatory mitigation for Section 404 waters, of the U.S. I,mpacts resulting from the project, then it is assumed the City will purchase mitigation banking crediits from an USACE-approved mitigation bank. 5. Geotec�hnic,al work will' include 6 borings of varying depth in the vicinity of the Weiler i headwall and downstream channel. An allowance for rock coring is included if required (see additional services category below). 6. Tree trimi,m;ing will be performed by the gieotechnical su�bcoinisu�ltant's personnel or their contractor, for work in and around backyards and' the existing channel. The subcontractor cost provided includes one (1) day of tree trimming and re ova of trimmings from sites. Tree trimming will be performed und�eir direction of the, City arborist. 7. Right of entry will be acquired by Jacobs and/or its subcontractors for access through the golf course and two private properties for g!eotechnical borings, surveys, and tree surveys. 8. The fee for geotechni'cal engineering services inc,ludes, costs for the drill crew to lay plywood sheeting progressively beineath drill equipment along access routes across the golf course and private properties to minimize rutting as the drill equipment travels across the golf course and two private properties for distances, of 200 feet to 350 feet; costs are provided biased on two days of drilling activity. Ruts, created by drilling operations will be repaired by a landscaper directed by the geotechn,ical engineer, with costs reimbursed by the City. An allowance of up to $5,000 is provided for rut repair work as additlioniall services (see additional services category below). 9 Urban Forestry Permit plan review fee, of$2510 wild not be paid by Jacobs. 10. Additional iterations of forestry permit will be considered additional services. D. Exclus"ions No fees have been provided for the items, listed below. Hydrologic and hydraulic modeling will be performed' only on existing and proposed storm drain infrastructure within the Eastern Hills watershed as described herein, in the original scople, and' in previous amendments. Should additional modeling be required using additional storms, on other waterways or within other watersheds that work will be accomplished as Additional Services, after approval by the City., 2. This scope does not include any review fees associate d with submittals to FEMA such as a Letter of Map Revision (L R); at present, the review fee for a LOMB of this nature (map change based on bridge, culvert, channel, or combination thereo0i is Updated: October 30, 2012 Page 7 Printed: 1/16/2013 1-058 PM I $5,300, and is subject to a yearly review and revision cycle. It is not expected that a Conditional Letter of Map Revision (CLOMR) will be required for this project. 3. Jacobs will not design any modifications or improvements to private structures such as sheds, that may be located within any existing or proposed easements. If such structures are affected: by any proposed improvements Jacobs will inform the City and: provide information about the nature of'the improvements at that location to the City's real prolperti�es, group. 4. Any other additional work not specifically included in the above scope of services will be accomplished as Additional Services, after approval by the City. 5. For the tree mitigation area, it is assumed that the contractor will be asked to submit an irrigation design including water and power supply as required for the trees and plants,, and that design for these items will not be included in the bid documents. IL AddItonal Serv*ices Fees are provided for the items listed below in case they are determined to be necessary. Compensation will be, provided on a time-and-materials basis according to the rates, personnel, and reimbursable costs detailed for each "Additional Services" item as detailed in Attachment B. 1. Fled plain-Development Permit: The proposed channel improvements lie within the regulatory (Zone A) FEMA flood lain of Shadow Creek, a tributary to Woodhaven Creek, as shown on panel 484391CO215K of the Tarrant County, Texas Flood Insurance Study, effective 9/2,5/20 9., Jacobs, will prepare and submit a flood lain development permit to the City of Fort Worth documenting the effects of the proposed channel improvements on the floodplain from the downstream face of Weiler to the tie-in with the Zone AE floodplain of Shadow Creek. No modeling or mapping on Shadow Creek is included or anticipated. Modeling prepared under item vi, Hydraulic Modeling, will be utilized to prepare, proposed floodpliain mapping exhibits and a short technical memo for review and approvai by City floodplain management staff. It is expected that a letter of map revision (LOMR) will ultimately be required for this area. Since a LOMR cannot be prepared and approved until after all construction "is, compilete, and construction is not currently scheduled to begin until Diecemlb!er 2013, the effort for preparation and submittal of the LO MR is included in this amendment as an additional service (see below). 2. Urban Forestry ,Permit: If needed, Jacobs will prepare and coordinate an Application for an, Urban Forestry Permit (Application) in accordance with, the City of Fort Worth Tree Ordinance requirements inclusive of the preparation of an Urban Forestry Plan Check List (Check List). Two (2) paper copies each of the Check List and Application will be provided to the Client. Data Collection,and Evaluation:Collect various information to be used for the preparation, of the Check List and Application. This will include conducting a tree survey of the, Project area which is comprised of approximately 1.7 acres. The 'tree survey will be performed in accordance with the City of Fort Worth Tree Ordinance requirements. It is estimated that approximately 40 — 50 trees will Updated: October,30, 2012 Page,8 Printed-, 1/16/2013 1:58 PM need to be evaluated and surveyed within the project area. Following the data collection effort, the information will be evaluated and organized for use in preparing the Check List and Application. Urban ,Forestry Plan Check List.,- In coordination with the Project Manager and Project Landscape Architect, prepare the draft Urban Forestry Plan Check List and provide the Client with one (1) hard copy for review and comment. Subsequent to receipt ofireview comments from the Client, prepare and transmit the final Check List for evaluation alnd processing. Perform on-going coordination with the City of' Fort Worth Arborist during the review/approval process. Application for Urban Forestry Permit.- In coordination with the Project Manager and Project Landscape Architect, prepare the Urban Forestry Permit Application and provide the Client with one (1) hard copy for review and comment. Subsequent to receipt of review comments from the Client, prepare and transmit the final Application for evaluation and processing. The Application will be submitted simultaneously with the Check List. Associated items to include, with the Application include two (2) full size Phase I (Existing Conditions Plan) and Phase II (Site Atterations/PlIanting Plan) Plan copies (tri-folded with name of project on copy face'- one (1) 8.5 x 11-inch mini neap of image area of plan; digital copies (CD or DVD) of the Phase I and Phase II Plansfo one kl) full size plat copy folded to City of Fort Worth standards. Perform on-going coordination with the City of Fort Worth Arborist during the review/approval process. i 3. Parks Conversion Permit: If needed, the parks, conversion permit work as described in the original scope of'work will be provided., 4. Tree Mitigation Pl.a. ntlin,g Plan If'neededi i, Jacobs will prepare a tree mitigation plan in association with the urban forestry permit and incorporate the plan into the design plans. The mitigation area is assumed to be available within the liimits of the area shown on Attachment A-1. 5. Rock Coring: An allowance for rock coring by the geotechnical engineering subconsultant is included, should subsurface conditions require. Costs for rock coring are $60.,00/foot if performed using portable equipment and/or $35.00/foot, if performed using a!, 4-w,heel drive all-terrain vehicle; the use of'a particular, piece of equipment is at the geotechnical eng:ineer"s discretion based on his professional judgment. 6. Tree Trim An allowance of $2,750 is included for tree trimmiing on private property, within the existing drainage easement, or on Meadowbrook Golf Course as may be required to obtain the geotechnical bores. Tree trimming wilil be performed by the geotechnical engineering subconsultant's personnel or their contractor,, for work in and around backyards and the existing channel. "rhe allowance cost provided includes one (1) day of tree, trimming and removal of trimmings from sites. Tree trimming, will be performed under direction of the City arborist. 7. Rut Rtes. While the geotechnical engineering sub con sultant is taking steps to reduce the risk of fire and rut damage to private property or Meadowbrook Golf Course, the possibility for such damage still exists. An allowance of up to $5,500 is provided for rut repair work as additional services. 6 8. After construction of these channel improvements is complete, Jacobs will obtain as-built survey data of'the improvements, provided by the contractor and/or City of Fort Worth, and use it to revise the previously-prepared HEC-RAS model to reflect the actual post-project conditions, of the channel and flooldpilain. The limits of this model and' floodplain revision are as described in section II 1. Since FEMA does not presently accept the InfoWorks software packages for map: revisions,, Jacobs will update existing models that are acceptable for FERIA submittals for the hydrology and existing hydraulics portions of the submittal. For hydrology, Jacobs will revise an existing HEC-HMS model, hich was originally prepared during the watershed study phase of this project., to accurately reflect watershed conditions at the time the Loll is prepared. For pre-project conditions hydraulics, Jacobs will update a H'EC-RAS model of the existing channel configuration, also prepared during the watershed study phase of'this project, extending it downstream to the limits described above and extending cross sections laterally as necessary to accurately model and map the fl'oodpllain. 9. Jacobs will prepare a LOMR Submittal package for the limits of the stream as described above, including all necessary information to reclassify the floodplain as Zone AE (detail study), including, 100-year and 500-year floodplains, base flood elevations FEs), and summary of discharge tables. This scope assumes that as fllooldwa will not be developed for this stream. The submittal package also includes notification letters to affected property owners and coordination with F'EMA reviewers, as necessary during the approval' process. Phase 1 Expanded Construction Admi-in-Istrati-on (CA), Services The scope/fee provided for Phase 1, services during construction included 8 site visits. As of the end of May 20,12, 1,9 visits had been provided As, of duly 2012, the contractor has installed topsoil and landscaping that does not appear to meet the standards, of material quality or installation as shown in the contract documents for this project. Beginning 'in June 2012, Jacobs engineers and landscape architects undertook an effort to provide expanded construction services to help identify and correct deficiencies with the installed landscaping, topsoil, misaligned maintenance access road, and other items as needled'.
