Contract No. CSC No. 54261 (the "Contract")
Renewal Term No._1_: _October 1, 2020 to_September 30, 2021
The above referenced Contract with the City of Fort Worth expired on September 30, 2020
(the "Expiration Date"). Pursuant to the Contract, the Contract may be renewed by the mutual,
written,consent of the parties. Though the parties did not process this renewal form until after the
initial term's expiration, it was the intent of the parties to renew the contract for its first renewal
period. And, in fact, the parties have treated the contract as though it had been renewed and
continued to perform under the terms therein. This renewal notice memorializes the parties intent
to renew the agreement for an additional one-year period,which began on October 1, 2020 and is
set to expire on September 30, 2021. In accordance with section 3 of the Contract,the City of Fort
Worth agrees to pay the Vendor the amount on the attached quote for this renewal period. All other
terms and conditions of the Contract remain unchanged. Please return this signed
acknowledgement letter, along with a copy of your quote for charges for the new renewal
term (current insurance certificate,to the address set forth below, acknowledging receipt of
the Contract Renewal Notice.
