Contract No. CSC No. 49578 (the "Contract")
Renewal Term No. 4: October 1, 2021 through September 30, 2022
The above referenced Contract with the City of Fort Worth expired September 30, 2021
(the "Expiration Date"). Pursuant to the Contract, contract renewals are at the sole option of the
City. This letter is to inform you that the City is exercising its right to renew the Contract for an
additional one (1) year period, which will begin immediately after the Expiration Date. All other
terms and conditions of the Contract remain unchanged. Please return this signed
acknowledgement letter, along with a copy of your quote for charges for the new renewal
term,and current insurance certificate,to the address set forth below,acknowledging receipt
of the Contract Renewal Notice. Failure to provide a signed acknowledgment does not affect
the renewal.

Quote for renewal of HSCloud Suite system for the City of Fort Worth
Environmental Health Department
This quote is for the renewal of services, for 12 months of continuing access, to all current
modules and features for the City of Fort Worth's HS Cloud Suite application:
This quote includes:
• Unlimited data storage, bandwidth, license, maintenance, and hosting, for the
City's current implementation - and all current features - of HSCloud Suite.
• Technical support via online portal, email, and phone for authorized City users.
• Continued ability, and support, to perform inspections in the field on Pads or
Windows 10 or higher devices (City chooses hardware). These devices have
functionality even when disconnected from the internet.
• Hosting and support of the online citizens portal allowing the City the ability to
display inspection results online for the general public, and accept complaints
online.
• Backup of data contained in City's HSCloud Suite implementation.
Time period of renewal:
The period of service for this renewal shall run from October 1 st, 2021 through
September 30th, 2022.
Costs:
The fees payable under this renewal shall be as follows:
• Cost for all services as defined above, for the period defined above, shall
be.
$36,000.00 (this includes unlimited users of the system for its intended
purpose and configured scope)
