CITY SECRETARY CONTRACT NO. 56367
COOPERATIVE PURCHASING AGREEMENT
This Amendment is entered into by and between the City of Fort Worth (hereafter
"City"), a home rule municipal corporation organized under the laws of the State of Texas, with
its principal place of business at 1000 Throckmorton Street, Fort Worth, Texas, and Intelicom
Inc. dba LiteSys Inc. (hereafter"Consultant"), an individual.
WHEREAS, the parties have previously entered into City of Fort Worth City Secretary
Contract No. 56367 (the "Contract"), which was executed on November 1, 2020; and
WHEREAS, the Contract involves professional services for assistance in preparation of
the City's Fiscal Year("FY") 2021 and FY 2022 Comprehensive Annual Financial Reports; and
WHEREAS,the original term of the Contract expires on October 31, 2021; and
WHEREAS, the City and Intelicom Inc. dba LiteSys Inc. now wish to amend the
original Contract to extend original term of Contract expiration from October 31, 2021 to
November 30, 2021.
NOW, THEREFORE, City and Consultant, acting herein by the through their duly
authorized representatives, enter into the following agreement to amend the contract:

3.0 TERM
The term of this Agreement shall be for one year beginning on November 1, 2020 ("
Effective Date") and shall expire on November 30, 2021(" Expiration Date"), unless terminated
earlier in accordance with this Agreement(" Initial Term"). Buyer shall have the option to renew
this Agreement under the same terms and conditions, for up to four ( 4) one- year renewal
options,by written agreement of the parties

All other provisions of the Contract which are not expressly amended herein shall remain
in full force and effect.
