WHEREAS, The City of Fort Worth and Elements of Architecture, Inc., (Consultant)
made and entered into City Secretary Contract No. 52790 (The Contract) which was
administratively executed the 16th day of September, 2019, for Police Department
Property/Evidence Room & Fire Department Arson Unit Area Build Out at Bob Bolen
Public Safety Complex — Schematic Design Phase Services; and Amendment No.1 was
authorized on September 1, 2020 by the City Council by M&C 20-0626 in the Amount of
$182,550.00 to provide design and construction administration services; and
WHEREAS, further amendment is necessary to reimburse the Consultant as part of the
design services agreement to revise drawings to reflect a new location for the proposed
Toilet Room.
NOW THEREFORE, City and Consultant, acting herein by and through their duly
authorized representatives, enter into the following agreement which amends the
Contract:

Article I — Scope of Services, Paragraph (3) shall be added to read as follows:
(3) Provide additional design services for the design and detailing of Build Out Area
in the existing Bob Bolen PSC. The cost of this additional service is $1,750.00
with no additional reimbursable expenses. The scope of services is described
more fully in Elements of Architecture, Inc. `s letter dated October 19, 2021
subject: "Proposal for A/E Services, Amendment #02 to Existing Contract, Bob
Bolen Renovations — Police and Fire Evidence and Storage Room".

Article II — Compensation, first paragraph, is amended as follows:
"Consultant shall be compensated in accordance with the Fee Schedule shown in
Attachment "D". Payment shall be considered full compensation for all labor,
materials, supplies, and equipment necessary to complete the services described
in Attachment "D". However the total fee paid by the City shall not exceed a total
of $219,300.00 unless the City and the Consultant mutually agree upon a fee
amount for additional services and amend this Agreement accordingly."

Bob Bolen Renovations— Police and Fire Evidence and Storage Rooms
Dear Alfonso;
Elements is pleased to provide to you this proposal for services associated with the addition of the restroom to be
included within the existing Bob Bolen Facility. This proposal is based on the scope determined and layout
approved. We have anticipated this change to be a revision to the documents already issued and that
construction of the work will be concurrent within the general construction for the remainder of the facility.
PROPOSED FEE
Labor
We propose to provide our services as identified in this proposal on a lump sum basis as follows:
Architectural $ 1,000.00
MEP Engineering $ 750.00
Total this task $ 1,750.00
Expenses
We believe that we have enough in expenses left within our original scope/fee to cover any expenses to be
incurred with this change.
