FIFTH AMENDMENT
TO FORT WORTH CITY SECRETARY
CONTRACT NO. 53147
This Fifth Amendment to City Secretary Contract No. 53147 ("Amendment") is entered
into by and between the City of Fort Worth("City"), a home rule municipal corporation organized
under the laws of the State of Texas, and Dr. Theron L. Bowman ("Consultant"), collectively the
"parties."
WHEREAS, on December 4, 2019, the parties previously entered into City of Fort Worth
City Secretary Contract No. 53147(the"Contract"),for the provision of consultant services related
to police interactions with the public;
WHEREAS, on February 6, 2020 the parties amended the Contract to allow Consultant to
engage a subcontractor;
WHEREAS, on November 11, 2020 and August 12, 2021, the parties agreed to second
and third amendments of the Contract to extend the term of the contract to November 1, 2021 and
expand the scope of services; and
WHEREAS, the parties wish to further amend the Contract to extend the term of the
Contract to January 16, 2022.
NOW, THEREFORE, Parties, acting herein by the through their duly authorized
representatives, enter into the following agreement to amend the Contract:
I.
The Parties agree to extend the Contract,under the same terms and conditions,with a new
expiration date of January 16, 2022, unless terminated earlier in accordance with the terms of the
Contract.
II.
All other provisions of the Contract which are not expressly amended herein shall remain
in full force and effect.
