HomeMy WebLinkAboutContract 54720-AD24 CSC No.54720-AD24 Addendum No. 24 Statement of Work This Addendum No.24- Statement of Work(SOW)is made as of this 18th day of November 2021, under the terms and conditions established in the MASTER PROFESSIONAL SERVICES AGREEMENT between the City of Fort Worth ("City") and Smith Temporaries, Inc. dba CornerStone Staffing ("Vendor"), the same being Fort Worth City Secretary Contract(CSC)No. 54720 (the"Master Agreement'' or"MSA"). This Statement of Work is made for the following purposes, consistent with the services defined in the Master Agreement: Providing temporary staffing services to the City of Fort Worth for the positions listed below. In particular, Vendor shall perform those functions identified below in the Scope of Services, as such services are more fully detailed in the MSA. Section A. Scope of Services & Schedule As a participating community of the National Flood Insurance Program(NFIP),the City is required to keep copies of all of the current and historical Flood Insurance Rate Maps (FIRMS). The newer maps are provided digitally but older FIRMS are paper copies that should be preserved digitally. Over the years, many engineering studies have been developed to establish and revise the FEMA regulatory floodplain shown on the FIRMS. These studies are leveraged by City staff and consulting engineers on a frequent basis. While the newer studies are easily located in a digital format most times, the paper copies (approximately 1980-2015) are much more cumbersome to locate and distribute. Once digitized, City staff and eventually the general public will be able to easily search and share all of the scanned data now located in the floodplain vault. Staff will be tasked with scanning physical file folders in the floodplain vault and organizing them in digital folders on TPW server. The folders to be scanned will mostly include flood studies with items such as engineering reports,maps,design plans, and digital data on CDs.The hard copy files will be scanned to PDF and eventually transferred into Laserfiche software. In addition to the scanning itself, staff will need to be able to interpret and recognize key data from the files such as stream names, engineering firm, Floodplain Development Permit number, and FEMA case number. Projected Start and End Dates for Assignment: December 1, 2021 —November 23, 2022 Anticipated Total# of Hours: 1400 hours for each position; 2,800 hours total Hourly Billing Rate (Must be in Exhibit B of the MSA): $24.08 Total Anticipated Cost: $67,424.00 OFFICIAL RECORD CITY SECRETARY FT. Compensation, Reimbursable Expenses, and Billing Terms: Processing and payment of fees, including the structure of payments and invoicing thereof, shall be in accordance with the Master Professional Services Agreement. The total fee that shall be paid by City to Vendor for these services shall be in accordance with the Master Service Agreement and, specifically,Exhibit"B"-Price Schedule. In submitting invoices, Vendor shall reference the appropriate Statement of Work. Further, the Vendor shall separately list the total amount for fees in which invoices were previously submitted during the then-current annual contract period (either initial term or renewal period) and the total amount of fees for the entire period, including those amounts being requested under the then-current invoice. Vendor shall submit invoices for Fees to Ben Thompson, within the Transportation and Public Works Department (Beni amin.Thompson(afortworthtexas.2ov)and also submit a copy to the Talent Acquisition Manager in the Human Resources Department, 200 Texas Street,Fort Worth,Texas 76102.
