Contract No. 51314 (the "Contract")
Renewal Term No. 3: October 1, 2021 to September 30, 2022
The parties executed the above mentioned Contract for its initial term of September 28,
2018 to September 30, 2019.
Pursuant to the Contract, the terms of the Contract renew automatically for up to three
one-year terms, if the City has appropriated sufficient funds. Though the parties did not execute a
formal renewal notice for the Contract's first renewal term, the City appropriated sufficient funds
and the Parties intended to renew the Contract for a first renewal term from October 1, 2019 to
September 30, 2020. And, in fact, the parties acted, and performed, as though the Contract had
been renewed. Therefore,the Parties agree that the Contract was renewed for its first renewal term
beginning on October 1, 2019 and ending September 30, 2020. Thereafter, the Parties executed a
formal notice for a second renewal term of October 1, 2020 to September 30, 2021. However, the
Parties erroneously titled the second renewal as the first renewal.This renewal notice is to formally
document that the Contract is being renewed for its third, and final, renewal term beginning
October 1, 2021 and continuing until September 30,2022.
All other terms and conditions of the Contract remain unchanged.
To agree to renew the Contract for an additional one (1)year period,please sign and
return this request to exercise renewal option, along with a copy of your current insurance
certificate, to the address set forth below.
