PROFESSIONAL SERVICES AGREEMENT
THIS RENEWAL OF CITY SECRETARY CONTRACT O, 42326 - PROFESSIONAL
SERVICES AGREEMENT ("Renewal") is made and entered into by and between THE CITY
OF FORT WORTH,, a home-rile municipal corporation of the State of Texas ("'City"), acting
by and through its duly authorized Assistant Ci

("Consultant" or `SUM"), an r di ideal..
WHEREAS, the City and C TY AS .NE']F, INC. entered into a Professional Services
Agreement for services related to sales tax analysis and reporting, the same being City Secretary
Contract No. 42326 ("'Contract"); and
WHEREAS, the City consented to the assignment of the Contract from
CITYBA� NET, INC. to the Consultant- and
WHEREAS, the Contract initially had an effective date of March 9, 2010 with a one-year
initial to , one automatic one-year renewal, and one, one-year optional renewal; and
WHEREAS, the City and Consultant entered into a.First Amendment and Renewal of the
Contract to exercise the optional ore-year renewal and to provide for a second ore-year renewal
option, that instrument being City Secretary Contract No. 42326-Al and
WHEREAS, the brat Optional Renewal Term expires March , 20131; and
WHEREAS, City and Consultant wish to renew and extend the contract for a second
{optional Renewal Term.
