EXTENSION OF COVID-19 VACCINATION SITE LICENSE AGREEMENT
BETWEEN THE CITY OF FORT WORTH
AND HERITAGE CHURCH OF CHRIST OF TARRANT COUNTY
THIS EXTENSION TO COVID-19 TESTING SITE LICENSE AGREEMENT ("Extension') is
made and entered into by and between the CITY OF FORT WORTH,a home rule municipal corporation
of the State of Texas ("Licensee" or "CjjZ") and and HERITAGE CHURCH OF CHRIST OF
TARRANT COUNTY,Texas Non-profit Religious Corporation,("Licensor').
WHEREAS, Licensor is the owner of certain property located at 4201 Heritage Trace Parkway,
Fort Worth,TX 76244; and
WHEREAS,in order to continue responding to the COVID-19 pandemic and its impacts,the City
requires additional outdoor and indoor space to install and operate temporary"pop-up"COVID-19 testing,
vaccination,and staging or support sites throughout the City; and
WHEREAS,in support of the City's mission to protect the health and safety of the ctizens of Fort
Worth and the surrounding area and to minimize the impacts of COVID-19, Licensor and City previously
entered into a COVID-19 Vaccination Site License Agreement dated May 10,2021 (City Secretary Contract
No. 55723)(the "Agreement")with a Term expiring on September 30,2021; and
WHEREAS, the Agreement allows the Term to be extended by the mutual written agreement of
the parties and City and Licensee wish to extend the Agreement through June 30,2022.
NOW, THEREFORE, in consideration of the covenants and agreements contained in the
Agreement,Licensor and Licensee hereby agree as follows:
1. Term
Unless terminated earlier pursuant to the terms of the Agreement,the initial term of the Agreement
shall be extended through June 30,2022.
2. Schedule
City shall use its best efforts to provide Licensor with one week's notice before commencing use
of the Licensed Premises.
3.
All other provisions of the Agreement shall remain in full force and effect.
