Renewal Term No. 3 —March 14, 2022 through March 13, 2023
The above referenced contract with the City of Fort Worth expires on March 13, 2022 (the
"Expiration Date"). Pursuant to the Contract, renewals are at the mutual agreement of the parties.
This letter is to inform you the City is requesting to renew the Contract for an additional one (1)
year period,which will begin immediately after the Expiration Date.All other terms and conditions
of the Contract remain unchanged.
To agree to renew the Contract for an additional one-year period,please electronically sign
and return this request to exercise renewal option. Also please send a copy of your current
insurance certificate to Mr.Roger Grantham,City of Fort Worth Environmental Supervisor
at the following email address: roger.granthamLar) ortworthtexas.gov.
Please log into your PeopleSoft Purchasing account with the City of Fort Worth at
www.hag.11fortworthtexas.gov/purchasing to ensure your company information is correct and
accurate and up to date.
