Premier Extension Amendment:
Microsoft Enterprise Services Work Order

This amends the Enterprise Services Work Order (the "Work Order") noted above, between Customer
and Microsoft and is effective as of the date that Microsoft signs this Amendment.
1. Amendment
Section 1.1, Term, of the Work Order is hereby extended, allowing additional time to complete your
renewal,for 30 calendar days (the "Extension Period").This Work Order Extension Period will expire on
5/30/2022 (the "Extension Period Expiration Date").The Work Order renewal Support Commencement
Date will be 5/1/2022.
Additional Terms that will apply to the Extension Period:
If all renewal documentation, including a purchase order, if applicable,or payment for the total amount
due, is received prior to the Extension Period Expiration Date, all Premier Support Services performed
during the Extension Period, including but not limited to hours for Problem Resolution Support and
Support Assistance,shall be deducted from the Services purchased under the renewed Support Services
plan.
If all renewal documentation is not received prior to the Extension Period Expiration Date, all Premier
Support Services will be terminated without further obligation to provide such Services. and we will
invoice you for Extended Services used during the Extension Period. Such invoice will be a fee based
on the Services and Extended Period Rates reflected below, plus an additional eight (8) percent of the
Extension Period amount.Our invoices are payable within 30 calendar days of the date of our invoice.

Up to 10 Problem Resolution Hours $ 260.00
Up to 16 Problem Resolution Hours $ 2S7.S0
Up to 4 Support Assistance $ 263.00
2. Effect of Amendment
Except as specifically amended by this amendment, all other provisions of the Agreement and
Work Order shall remain unchanged, and in full force and effect.
