FIRST AMENDMENT TO
FELLOWS AGREEMENT (CSC# 57346)
This First Amendment to Fellows Agreement ("First Amendment") is made between the
City of Fort Worth("Fort Worth")and FUSE Corps,a California non-profit corporation("FUSE").
WHEREAS, Fort Worth and FUSE entered into an Agreement identified as Fort Worth
City Secretary Contract No. 57346 beginning April 4, 2022 (the"Agreement"); and
WHEREAS, it is the collective desire of both Fort Worth and FUSE to amend the
Agreement to extend the termination date.
NOW THEREFORE, known by all these present, Fort Worth and FUSE, acting herein by
the through their duly authorized representatives, agree to the following terms, which amend the
Agreement as follows:
1. The Agreement is hereby replacing Section 2. Term and Effective Date in its
entirety with the following:
Term and Effective Date. The Term of this Agreement shall commence upon full
execution of this Agreement, and expire on July 24,2023,unless earlier terminated
by the Parties in accordance with the terms of this Agreement or unless extended
through an amendment(the "Term").
2. The Agreement is hereby amended by replacing the Tables in Attachment 1 for the
Diversity and Inclusion Department and the Economic Development Department with the
following tables,respectively:

December 1, 2021 Agreement between FUSE and City Department
to host an Executive Fellow in the upcoming
program year
April 25, 2022 Payment of$12,500 total fee is due
April 25, 2022
Week-long Virtual Opening Leadership Institute
May 2, 2022 Executive Fellow's first official day working
onsite, including structure onboarding actions
directed by the Fellow's director report
April 24, 2023 Final day of work for Executive Fellow(unless
agreement is extended

3. All other terms,provisions,conditions, covenants and recitals of the Agreement not
expressly amended herein shall remain in full force and effect.
