PROJECT: Stage Coach Ground Storage Tank Rehabilitation Project No 02316
The following is a Contract Modification Request (CMR) from elastco, Inc,, for repairs required to correct existing
corroslon and damage to the interior center rafters and support:

Install a center column support anci halo, complete and in place; 1 $14,905.00
Remove the existing corroded rafter sections and plate, similar to the
detail shown in the attached drawing. The proposed price is based on
the cost breakdown as shown in the attached table. Contractor shall
submit a detail specific for this project that is signed and sealed by a
State of Texas licensed engineer for review by the Owner, prior to
commencement of work.
Time Extension: An additional 10 calendar days will be added to tf�e Contract Scl�edule to complete the proposed
repairs.

10. INFORMATION F2EQUIRED: Upon inspection after the initial blast operation, excessive corrosion was found
on the rafters at the center support structure. How would the City of Fort Worth like us to proceed? Blastco
has inciuded a couple options for repairing the support structure. Please see the proposal attached for more
information.

We are pleased to provide a proposal for the above project.
A. Scope of Work — New Center Support Structure
• Provide materials, labor, and equipment to rernove existing center colurnn dollar plate, cut
back existing rafters, and install a new a' diarneter halo support structure to replace the
existing center support structure that has deteriorated due to corrosion. See attached repair
detail from another project and photos for reference.
Pricing: $15,500.00
B. Scope of Work — Modify Existing Center Support Structure
• Cut out the center section of the roof above the center support.
• Cut back corroded rafters to good steel. �
• Re-weld cut out roof section back in place.
Pricing: $7,500.00
