AMENDMENT No. 1
TO CITY SECRETARY CONTRACT No. 54162
WHEREAS, the City of Fort Worth (CITY) and Distinctive AFWS Designs,
Inc. , (CONTRACTOR) made and entered into City Secretary Contract No.
54162, (the CONTRACT) which was authorized by M&C 20-0400 (June 16, 2020)
in the amount of $1, 196, 001 . 00; and
WHEREAS, the CONTRACT involves professional services for the
following project:
Assessments and Installation Services for High Water Warning System
Conversions; and
WHEREAS, due to the disruptions to supply chains and equipment
availability, it is necessary to update the pricing of certain fees as
well as to add additional line items so that CONTRACTOR can adequately
perform the work; and
WHEREAS, due to Covid-19 related delays during the initial term of
the contract, it has become necessary to extend the contract' s term.
NOW THEREFORE, CITY and CONTRACTOR, acting herein by and through
their duly authorized representatives, enter into the following
agreement, which amends the CONTRACT:
1 .
Article 1 . of the CONTRACT is amended to include the additional
services specified in Attachment "A" attached hereto and incorporated
herein. The CONTRACT cost to City is not changed.
2 .
Article 2 . of the CONTRACT is amended to extend the term from the
sooner of exhaustion of funds or July 16, 2022, to December 31, 2022 or
the expenditure of the funds, whichever occurs later.
3.
All other provisions of the Contract, which are not expressly
amended herein, shall remain in full force and effect.
