HomeMy WebLinkAboutContract 55589-A1 CSC No. 55589-Al FIRSTAMENDMENT TO CITY SECRETARY CONTRACT NO. 55589 This Amendment ("Amendment") is made and entered into by and between the City of Fort Worth ("City") and Camp Fire First Texas ("Agency") by through their duly authorized agents. City and Agency may be referred to individually as "Party" and jointly as "Parties." WHEREAS,the City of Fort Worth("City") and Camp Fire First Texas ("Agency")made and entered into City Secretary Contract No. 55589 ("Contract''); WHEREAS, City receives grant monies from the United States Department of Housing and Urban Development (HUD) through the Community Development Block Grant (CDBG) Program, Program No. B-20-MW-48-0010, Catalog of Federal Domestic Assistance No. 14.218; WHEREAS, City has received an additional allocation of CDBG grant monies under the Coronavirus Aid, Relief, and Economic Security Act ("CARES" Act), referenced by HUD as "CDBG-CV3"; WHEREAS,pursuant to Section 3 of the Agreement,the contract may be extended if such extension is necessary for completion of the program and is authorized, and the Agency is requesting an amendment to extend the term of the agreement through September 30, 2022, as reflected in the attached amended Exhibit A; WHEREAS, City believes that the requested amendment will help Agency meet the program's goals and further the interests of both Agency and City; and WHEREAS,it is the mutual desire of City and Agency to amend the Contract to complete the Program and meet Contract objectives. NOW,THEREFORE, City and Agency hereby agree to amend the Contract as follows: I. EXHIBIT "A" — PROGRAM SUMMARY attached to the Contract is hereby deleted and replaced in its entirety with EXHIBIT"A"Revised 08/10/2022, attached hereto. II. This amendment is effective as of July 1, 2022 and Agency may request reimbursement for allowable costs directly related to implementation CDBG CV3 funds. OFFICIAL RECORD CITY SECRETARY FT. WORTH, TX Camp Fire First Texas (CDBG-CV3) Page 13 CAMP FIRE FIRST TEXAS EXHIBIT "A" AFTER SCHOOL PROGRAM PROGRAM SUMMARY: CDBG CV3 JANUARY 1,2021-SEPTEMBER 30,2022 $120,000.00 PERIOD AMOUNT Capitalized terms not defined herein shall have meanings assigned them in the contract. PROGRAM: The program will provide after school and summer break programming to children of Fort Worth residents who are impacted by COVID-19.The goal of the program is to allow parents to continue to work while their children thrive in a safe and supportive out-of-school learning environment. The Program responds to the COVID-19 Pandemic by: • Prevention: providing a safe, healthy, masked, socially distanced and sanitized environment and providing separate STEM and art kits for each child. • Preparation: including information about the virus, infectious diseases and how they spread, the importance of washing hands and wearing masks in STEM lesson plans. • Responding: taking temperatures of the children daily upon entrance to the facility, isolating children who may become ill during the day,keeping children from the same families together in a cohort,adding additional staff to provide socially distanced 1:10 adult to child ratios. Eligible clients are defined as children age 4— 13,who are residents of the City of Fort Worth.Participants will be required to provide a completed and signed City of Fort Worth Certification of Income Statement to confirm eligibility. CDBG-CV funds will be used to pay for direct costs of delivering the program, including salaries and benefits for additional staff to maintain 1:10 adult to child ratios,bus driver to transport children to Diamond Hill Station, PPE,sanitizing supplies and individual STEM and art supply kits for each child. Agency shall not change any locations without prior City approval. Costs incurred at any locations that have not been approved by City shall not be reimbursed. PROGRAM GOALS: Minimum Number of Fort Worth Clients to be Served: The Program will serve a minimum of 120 Unduplicated Clients from Fort Worth.Agency is a Countywide Agency that provides services to Fort Worth and Non-Fort Worth clients through Fort Worth programs and a Non-Fort Worth program.Non-Fort Worth clients constitute at least 10% of total clients served. CDBG-CV3 funds serve Fort Worth clients only, and Agency will submit documentation verifying that 51% of Program clients are Income Eligible Clients with current household incomes at or below 80% of AMI. Agency will also submit quarterly reports as required through EXHIBIT"A-2"—Reporting Requirements for Countywide Agency Programs.
