SECOND AMENDMENT TO
CITY SECRETARY CONTRACT NO. 53044
This Amendment is entered into by and between the City of Fort Worth (hereafter
"City"), a home rule municipal corporation organized under the laws of the State of Texas, with
its principal place of business at 200 Texas St, Fort Worth, Texas, and Cindy Dickason d/b/a
PSYCHSCREENING (hereafter "Vendor").
WHEREAS, City and Vendor entered into an Agreement identified as Fort Worth City
Secretary Contract No. 53044, which was executed on November 11, 2019 (the "Agreement");
and
WHEREAS, City and Vendor amended the Agreement to include the annual price for
renewal period, which was executed on September 17, 2020; and
WHEREAS, City and Vendor amended the Agreement to include Detailed Scope of
Services, Relevant Laws, Job Description and TCOLE Form, which was executed on September
17, 2020; and
WHEREAS, City and Vendor amended the Agreement to redefine Vendor as "Cindy
Disckason d/b/a PSYCHSCREENING", which was executed on September 17,2020; and
WHEREAS, City and Vendor now wish to amend the Agreement to increase the annual
not to exceed amount from $40,000.00 to $70,000.00
NOW, THEREFORE, City and Vendor, acting herein by the through their duly
authorized representatives, enter into the following agreement to amend the contract:
1. Fee The annual renewal price shall not exceed Seventy Thousand Dollars ($70,000.00).
2. All other provisions of the Contract which are not expressly amended herein shall remain
in full force and effect.
