Renewal Term No. 2: July 9, 2023 to July 8, 2024
The above -referenced Contract with the City of Fort Worth expires on July 8, 2023
("Expiration Date"). The Contract includes three (3) one-year options for renewal. This letter is to
inform you that the City is requesting to renew the Contract for an additional one (1) year period,
which will begin immediately after the Expiration Date. All other terms and conditions of the
Contract remain unchanged.
To agree to renew the Contract for an additional one (1) year period, please sign and
return this request to exercise renewal option, along with a copy of your current insurance
certificate, to the address set forth below.

that your company information is correct and up-to-date.
