AMENDMENT NO.4
TO CITY SECRETARY CONTRACT NO. 53726
PROFESSIONAL BUILDING INSPECTION AGREEMENT
This AMENDMENT TO CITY SECRETARY CONTRACT NO. 53726,
PROFESSIONAL BUILDING INSPECTION SERVICES AGREEMENT is made and
entered into by and between the City of Fort Worth (hereafter "City"), a home rule municipality,
with its principal place of business at 200 Texas Street, Fort Worth, Texas, acting herein by and
through Dana Burghdoff, its authorized Assistant City Manager, and SAFEbuilt Texas, LLC, a
Texas limited liability company, and acting by and through its duly authorized president. City
and Company shall be referred to individually as a Party and collectively as the Parties.
WHEREAS, the City and Company entered into a professional building inspection
agreement for inspection services for projects, on an as needed basis, for all trades, including
building, electrical, mechanical, plumbing, and energy, City Secretary Contract Number 53726
("Agreement"); and
WHEREAS, the Agreement provides for three (3) one-year renewal terms; and
WHEREAS, City and Company executed the First Amendment to the Agreement to
renew the Agreement for the first renewal term, City Secretary Contract Number 53726-A1; and
WHEREAS, City and Company executed the Second Amendment to the Agreement to
renew the Agreement for the second renewal term, City Secretary Contract Number 53726-A2;
and
WHEREAS, City and Company executed the Third Amendment to the Agreement to
increase the maximum amount of compensation City may pay Company pursuant to terms and
conditions of the Agreement, City Secretary Contract Number 53726-A3; and
WHEREAS, the City and Company wish to amend the Agreement to exercise the option
to renew the Agreement under the same terms and conditions for a third and final renewal term,
commencing on the date the City and the Company have executed this amendment.
NOW, THEREFORE, the Parties, acting herein by the through their duly authorized
representatives, enter into the following agreement:

AMENDMENTS
1. City an Company hereby agree, in accordance with Section 2.01, Term, of the Agreement
to renew and extend the term for a renewal period of one-year, commencing upon the date the City
and Company have executed this amendment.
2.
ALL OTHER TERMS SHALL REMAIN THE SAME
All other provisions of City Secretary Contract No. 53726 which are not expressly
amended herein shall remain in full force and effect.

This Amendment may be executed in multiple counterparts, each of which shall be
an original and all of which shall constitute one and the same instrument. A facsimile copy
or computer image, such as a PDF or tiff image, or a signature, shall be treated as and shall
have the same effect as anoriginal.
