Renewal Term No. 1: May 1, 2023 to April 30, 2024
The above referenced Contract with the City of Fort Worth expired on April 30, 2023
while awaiting the finalization of the company's name change. Pursuant to the Contract, contract
renewals are at the sole option of the City. This letter is to inform you that the City is exercising
its right to renew the Contract for an additional one (1) year period, which will begin
retroactively on May 1, 2023. All other terms and conditions of the Contract remain unchanged.
Please return this signed acknowledgement letter, along with a copy of your current
insurance certificate, to the address set forth below, acknowledging receipt of the Contract
Renewal Notice. Failure to provide a signed acknowledgment does not affect the renewal.

EXPLANATION
Attached is the First Renewal of CSC 57963, the agreement between the City of Fort Worth
Water Department and C1iniSys, Inc. (C1iniSys). This agreement provides software, licensing,
maintenance, support, training and professional services for the Water Lab's use of the C1iniSys
Laboratory Platform, which prior to the company changing hands, was known as the Horizon
Laboratory Information Management System (LIMS).
This renewal was delayed while a Consent of Assignment name change document was processed
by Finance, approved by the Vendor, and filed with the City Secretary. The maintenance &
support renewal fee has already been processed as a non -PO voucher, while we worked on the
name change and the subsequent renewal.
This contract renewal is urgent due to the pending system configuration of the Lab
platform that is necessary in order to support new PFAS monitoring that is a new
regulation.
This renewal term runs from May 1, 2023 through April 30, 2024. The "not to exceed" amount is
$79,529.00.
