RENEWAL NO.3 TO
CITY OF FORT WORTH CONTRACT 54693
This Renewal is entered into by and between the City of Fort Worth ("City"), a home -
rule municipality, and UNIQUE MANAGEMENT SERVICES, INC. ("Vendor"), each
individually referred to as a "Party" and collectively referred to as the "Parties."
WHEREAS, on October 15, 2020, Parties entered into City Secretary Contract No.
54693 ("Agreement");
WHEREAS, Agreement was executed for Vendor to provide City with library fine
collection, library material recovery, and patron notification services ("Agreement");
WHEREAS, the original term of Agreement was for one year with unlimited one-year
renewals;
WHEREAS, Parties wish to renew the term of the Agreement for continued services
pursuant to the original Agreement for an additional year, specifically from August 4, 2023 to
August 3, 2024.
NOW, THEREFORE, Parties, acting herein by the through their duly -authorized
representatives, enter into the following agreement:
1.
RENEWAL OF AGREEMENT
The term of the Agreement is hereby renewed for an additional year to begin on August 4, 2023
and expire on August 3, 2024, unless terminated earlier in accordance with the Agreement.
The attached document from Vendor is hereby attached and incorporated for all purposes as
Attachment A for the purpose of documenting a rate increase.
2.
ALL OTHER TERMS SHALL REMAIN THE SAME
All other provisions of the Agreement which are not expressly amended herein shall remain in full
force and effect.
3.
ELECTRONIC SIGNATURE
This Renewal may be executed in multiple counterparts, each of which shall be an original
and all of which shall constitute one and the same instrument. A facsimile copy or computer
image, such as a PDF or tiff image, or a signature, shall be treated as and shall have the same
effect as anoriginal.

RE: Notice Outsourcing Service Price Increase
Dear Mr. Shidal,
The Fort Worth Library has been outsourcing printed bill notifications with Unique
for 10 years. It is a privilege working alongside your team for so many years!
Due to the rising costs of material, labor, and postage, we must inform you that
Unique will be increasing the price for print notifications, effective July 1, 2023. The
price per notice will increase from the current rate of $0.753 to $0.79. This increase
only applies to print notifications and will not impact the prices of phone, SMS, and
email notifications.
We remain committed to providing you with the highest quality of service and the
best value for your investment. We understand that price is an important factor for
our customers and strive to keep it as low as possible. Despite the price increase, we
remain dedicated to maintaining low prices with the same high quality and value.
We want to thankyou for your continued patronage and support, and we are
available to answer any questions or concerns you may have about the price
increase. Additionally, if you are interested in learning more about outsourcing
phone, SMS, and/or email notices with Unique, please let me know. The price change
will be reflected on the library's invoice dated August 1, 2023.
