COUNTY OF TARRANT S
WHEREAS, the City of Fort Worth (City) and Teague Nall and
Perkins, Inc. (Engineer) made and entered into City Secretary
Contract No. 27966, (the Contract) which was authorized by the
City Council by M&C C-19206 on the 27t' day of August 2002 and
subsequently revised by three previous amendments; and
WHEREAS, the Contract involves engineering services for the
following project:
Sanitary Sewer Rehabilitation Contract LII (52) .
WHEREAS, it has become necessary to execute Amendment No. 4
to said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and
through their duly authorized representatives, enter into the
following agreement which amends the Contract:

Article I of the Contract is amended to include additional
engineering services specified in a proposal letter dated March
30, 2011, a copy of which is attached hereto and incorporated
herein. The cost to City for the additional design services to be
performed by Engineer total $3,600.40.

Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work
and services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not
exceed the sum of $428,289.00.

All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effect.

Dear Mr. Chaubey:
Teague Nall and Perkins, Inc. is submitting this fee Amendment for the above referenced contract
based on additional services required by the City The following is a brief description and cost for
the additional services that we are providing on this project:
Additional Design Services
The City of Fort Worth Water Dept. informed TNP at the June 1, 2010 consultants'quarterly
project status meeting that the SS Rehab projects remaining for bid were being considered
for State Revolving Fund (SRF) Tier II loan funding. As a result of this consideration, the
Water Dept. requested TNP to submit the following exhibits: 1) USGSquad map indicating
the locations of the proposed sanitary sewer improvements2)description table identifying
the SS laterals and whether they were relocation or replacement lines.
Per direction from the Water Dept., he replacement lines in existing alignment will be funded
by SRF and the relocated lines will be funded by the operating budget. To clarify this funding
scenario, the Water Dept. requested TNP to modify the construction plan set and spec book
to identify and label the replaced lines in existing alignment as Unit 1 and the relocated lines
as Unit 2. This work involves labeling SS lines as Unit 1 or Unit 2 in the layout sheets and
plan & profile sheets. The work also consists of adding specific TWDB (SRF Tier II Funding)
language to the contract spec book, as well as, breaNng out the proposal bid items Into Units
1 and 2 in the contract spec book
The following is a breakdown of additional costs incurred for the expansion in scope.
Label SS lines in plans, modify spec book and proposal....................$ 2,880
(Incktdes modified spec book and proposal-2 days each for cad tech and Ell)
Prepare TWDB quad map and table description.................................$ 720
(0.6 day each for cad tech and Eln
