WHEREAS, the City of Fort Worth (City) and Teague Nall and
Perkins, (Engineer) made and entered into City Secretary Contract
No. 27966, (the Contract) which was authorized by the City Council
by M&C C-19206 on the 27th day of August 2002 and subsequently
revised by four previous amendments; and
WHEREAS, the Contract involves engineering services for the
following project:
Sanitary Sewer Rehabilitation Contract LII (52) .
WHEREAS, it has become necessary to execute Amendment No. 5
to said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and
through their duly authorized representatives, enter into the
following agreement which amends the Contract:
1.
Article I of the Contract is amended to include additional
engineering services specified in a proposal letter dated June 9,
2011, a copy of which is attached hereto and incorporated herein.
The cost to City for the additional design services to be
performed by Engineer total $13,000.00.

Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work
and services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not
exceed the sum of $441,289.00.

3.
All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effect.

Dear Mr. Chaubey:
Teague Nall and Perkins, Inc. is submitting this fee amendment for Contract 52 Part 3 based on
additional effort required by the City but not included in the original scope of work. The following is a
brief description and cost for the additional services that we are providing on this project:
Additional Design Services
The City of Fort Worth Water Dept. entered into an agreement with TNP in August of 2009
for the design of sanitary sewer line M-39 near the intersection of US 287 and Lancaster
Avenue. This agreement was based on sanitary sewer alignment option 'B' (see attached
Option 'B' exhibit). TNP proceeded with conceptual and preliminary design using this
alignment. In June 2010, it became necessary for the City to modify the proposed alignment
(see attached revised alignment). This change required changes to the engineering design
and required coordination with a consultant engaged in an adjacent project.
The following is a fee breakdown for the additional services:
