FIRST AMENDMENT TO
FORT WORTH CITY SECRETARY CONTRACT NO. 55627
This Amendment is made between the City of Fort Worth ("Fort Worth") and Sharon
Dolak ("Vendor").
WHEREAS, Fort Worth and Vendor entered into an Agreement identified as Fort Worth
City Secretary Contract No. 55627 beginning April 23, 2021 (the "Agreement"); and
WHEREAS, it is the collective desire of both Fort Worth and Vendor to amend the Price
Schedule under the Agreement to add a fee related to cancelations.
NOW THEREFORE, Fort Worth and Vendor, acting herein by the through their duly
authorized representatives, agree to the following terms, which amend the Agreement as follows:
1. Exhibit B of the Agreement, Price Schedule, is hereby deleted in its entirety and
replaced with the attached, revised Exhibit B.
2. All other terms, provisions, conditions, covenants and recitals of the Agreement not
expressly amended herein shall remain in full force and effect.

PRICE SCHEDULE
Summary of Costs Price
Mediation Fee: $125.00 an hour
Annual Cost (not to exceed): $30,000
• Expenses include consultant travel, postage/shipping, telephone,
support services, copying and other preparation fees, etc.
Memorandum of Understanding: $100.00
Cancelations: $75
• For each team seeking Mediation, the first cancellation/no show
under 48 hours is at no cost, but any future cancellation/no show
under 48 hours would be $75 per occurrence.
Total:
Per hour basis $125.00**
