WHEREAS, the City ,of Fort Worth (City) and Wade & Associates,
Inc. , (Engineer) made and entered into City Secretary Contract No.
28404, (the Contract) which was authorized by the City Council by
M&C C-19437 on the 21st day of January 2003; and
WHEREAS, the Contract involves engineering services for the
following project:
Engineering contract for the design of Sanitary Sewer
Rehabilitation Contract LVI (56) .
WHEREAS, it has become necessary to execute Amendment No. 1
to said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE; City and Engineer, acting herein by and
through their duly authorized representatives, enter into the
following agreement which amends the Contract:
1.
Article I of the Contract is amended to include the
additional engineering services specified in a proposal letter
dated December 13, 2004, a copy of which is attached hereto and
incorporated herein. The cost to City for the additional design
services to be performed by Engineer total $22, 916.50 .
2.
Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work
and services performed under the Contract, as amended, so that- tho
total fee paid by the City for all work and services shall not
exceed the sum of $312, 042 .50.

Per your instructions, we have revised our Contract Amendment No. 1 request to reflect the
appropriate changes. The revised amendment request for this agreement addresses additional
engineering services that were previously authorized by the Fort Worth Water Department on the
above referenced project for sanitary sewer improvements in the Henderson and W. Belknap
Streetarea.
The original scope of world required the re-route L-39 from its present location in an easement
between Belknap and Weatherford Streets. Our conceptual plan for L-39 intended to locate the
new sewer alignment in Belknap St. During the formal review meeting, we were asked to
modify this plan to move the proposed sewer line into Weatherford St. Once preliminary plans
were produced for this location, a field visit was conducted to examine other area sewer
improvements being designed to serve a new development project. Based on the field visit, we
were instructed to move the proposed L-39 realignment back to our original Belknap location.
The overall design revisions for this location resulted in the collection of additional topographic
survey data for Weatherford alignment and,it.required us to address the downstream portion of
L-39 from Sta. 5+04 to the M-15 R improvements, east of the Weatherford Traffic Circle. As a
result, the overall design footage for L-39 increased from 466 LF to 1,084 LF. Therefore, the
design footage was increased by 618 LF for L-39 and approximately 274 LF of 6-inch sewer
service relocations were also designed at three locations.
Additionally, we were also instructed to address the rehabilitation design of L-44 in Henderson
St. from Sta. 10+95E to 1+73 at this time. Sewer line L-44 was not included in the original
scope of work,but it was added to facilitate there-route of L-39 and complete the sewer system
improvements in this area. The addition of L-44 increased the design footage by 490 LF for this
project: This location also required the design of 17 LF for a 6-inch sewer service-relocation:
Therefore, the Henderson/Belknap St. area sewer improvements have resulted in a total~sewer
design footage increase of 1,109 LF for this project location. A total of 291 LF of 6-inch service

laterals were designed,at 4 locations. This project also included the design of four extensive 4-
inch service lateral relocations totaling approximately 478 LF. Finally, the design changes
associated with this area resulted in the collection of 845 LF of additional survey footage in
Weatherford and Henderson Streets.
Please note that we may seek additional funding in the future to cover the labor costs associated
with processing the additional topographic survey data that was collected for the alignment
change(Weatherford St.) and the portion of L-44 that was abandoned in Henderson St.

Therefore, we are requesting Contract Amendment No. 1 to the Contract LVI(56)Rehabilitation
and Improvements Agreement in the amount of $22,916.50. This amendment represents
approximately an eight percent (7.93%) increase in the total contract value. For your
convenience, a summary of the total project fees are as follows:
