AMENDMENT 1, RENEWAL 1 TO
FORT WORTH CITY SECRETARY CONTRACT NO.36591-A3AD1
This Amendment and Renewal to Fort Worth City Secretary Contract No. 36591-A3AD1
is made between the City of Fort Worth ("Fort Worth") and the Law Offices of Ricky D, Green
("Vendor").
WHEREAS, Fort Worth and Vendor entered into an Agreement identified as Fort Worth
City Secretary Contract No. 36591-A3AD1 beginning June 3, 2019 (the "Agreement") for Vendor
to provide City with worker compensation services, as outlined in City Secretary Contract No.
36591-A3;
WHEREAS, the Agreement expires on November 1, 2023 but allows for two one-year
renewal options at City's discretion;
WHEREAS, it is the collective desire of both Fort Worth and Vendor to amend the
Agreement to update the language under Section 6.2 of the Agreement related to annual payment
in order to increase the amount from Nine Thousand Six Hundred Dollars to Fourteen Thousand
Four Hundred Dollars; and
WHEREAS, it is the collective desire of both Fort Worth and Vendor to renew the
agreement for its first renewal term, beginning November 2, 2023 and ending November 1, 2024.
NOW THEREFORE, Fort Worth and Vendor, acting herein by and through their duly
authorized representatives, agree to the following terms, which amend and renew the Agreement
as follows:
1. Section 6.2 of the Agreement is hereby amended by deleting the section in its
entirety and replacing it with the following language: "The total of all payments
and obligations made and incurred by City hereunder, in consideration for services
rendered by Carrier Rep, shall not exceed the amount of Fourteen Thousand Four
Hundred Dollars and No Cents ($14,400.00) annually or during any Renewal
Term."
2, The Agreement is hereby renewed for its First Renewal Term, beginning November
2, 2023 and ending November 1, 2024.
3. All other terms, provisions, conditions, covenants and recitals of the Agreement not
expressly amended herein shall remain in full force and effect.
