In accordance with the Professional Services Agreement signed between Matri x
Resources Inc . (Contractor) and the City of Fort Worth (Client) on October 2 , 2009 Cit y
Secretary Contract ("CSC ") 39475 , as amended by CSC No. 39475-A 1, on Septembe r
13 , 2010 (collectively the "Profess ional Services Agreement"), Append ix B-6A , 1s
incorporated into the Professional Services Agreement and hereby read as fo ll ows:
1. The term of this work order shall be extended to end when all services have
been completed under this work order as contemplated herein , but not la ter
than the end date set forth below .
2 . This Work Order incorporates by reference all of the terms and conditions of
the Professiona l Services Agreement , spec ifically the serv ices to be pro v ide d
under Section 1 of said agreement
3 . The services to be performed under th is Work Orde r shal l be as fo l lows
HELP DESK AGENT (Computer Help Desk Environment)
To troubleshoot , modify and correct MS Exchange ema il bo x issues . To provide gene ra l
computer customer service support and information in operating , using and maintaining
computer systems and applications ; to conduct various techn ica l tasks related to the
operation and maintenance of information systems and equ ipment. Troubles hoot
hardware/software problems for failure determination . Create/mod ify/remove network
and mainframe security accounts .
