In accordance with the Professional Services Agreement signed between Matrix
Resources Inc . (Contractor) and the City of Fort Worth (Client) on October 2 , 2009 City
Secretary Contract 39475 , as amended by CSC No. 39475-A 1 on September 13 , 2010
(collectively "Professional Services Agreement"), Appendix B-8 , is incorporated into the
Professional Services Agreement and hereby read as follows :
1. The term of this work order shall be extended to end when all services have
been completed under this work order as contemplated herein , but not later
than the end date set forth below .
2 . This Work Order incorporates by reference all of the terms and conditions of
the Professional Services Agreement , specifically the services to be provided
under Section 1 of said agreement.
3. The services to be performed under this Work Order shall be as follows :
HELP DESK AGENT (Computer Help Desk Environment)
To troubleshoot , modify and correct MS Exchange email box issues. To provide general
computer customer service support and information in operating , using and maintaining
computer systems and applications ; to conduct various technical tasks related to the
operation and maintenance of information systems and equipment. To troubleshoot
hardware/software problems for failure determination . To create/modify/remove network
and mainframe security accounts
