FIRST AMENDMENT TO CITY SECRETARY CONTRACT NO. 39329
PROFESSIONAL SERVICES AGREEMENT

This First Amendment to City Secretary Contract No. 39329 - Professional Services Agreement ("First
Amendment") is made and entered into this V St- day ofjPeceAjdec, 2011, by and between the City
of Fort Worth ("CITY'), a home -rule municipal corporation of the State of Texas, acting by and through its
duly authorized Assistant City Manager, and Occupational Health Centers of the Southwest, P.A., DBA
Concentra Medical Centers ("CONCENTRA'), a Texas Professional Association, acting by and through
its duly authorized Senior Vice President and Chief Medical Officer.
WITNESSETH:
WHEREAS, CITY and CONCENTRA have entered into a Professional Services Agreement
effective October 1, 2009, the same being City Secretary Contract No. 39329 (the "Agreement"), and
WHEREAS, pursuant to the Agreement, CONCENTRA provides non -occupational -injury health
care services to CITY employees, including drug screenings, physician assessments, and vaccinations,
with such services being performed primarily at designated Fort Worth locations during specified operating
hours; and
WHEREAS, CONCENTRA has notified the CITY of a change in location and operating hours for
some of the specifically designated locations; and
WHEREAS, CITY and CONCENTRA wish to amend the Agreement to reflect the revised location
and operating hours; and
WHEREAS, CITY and CONCENTRA also wish to amend the Agreement to allow for the provision
of and compensation for additional screenings, assessments, and vaccinations.
NOW THEREFORE, in consideration of the foregoing and the mutual covenants and agreements
contained herein, CITY and CONCENTRA hereby agree as follows:
1. That Section 1, "Concentra Locations/Facilities and Hours of Operation," of Exhibit A, "Scope of
Services" is hereby deleted in its entirety and replaced with the following:
CONCENTRA LOCATIONS/FACILITIES AND HOURS OF OPERATION
CONCENTRA operates fifteen (15) clinic locations in the Fort Worth/Dallas area and 300 other locations
around the United States. The CITY will primarily utilize CONCENTRA's Fort Worth locations as seen
below, but can access any other CONCENTRA location as necessary:

CITY employees will have their own entrance to the Fort Worth/Forest Park Clinic. CONCENTRA
will dedicate the back half of the Fort Worth/Forest Park Clinic for the CITY services and needs,
including a dedicated telephone number (817) 335-3320, a CITY check -in desk, a private waiting
room, and two CITY dedicated exam rooms.
All other portions of Exhibit A, "Scope of Services," of City Secretary Contract 39329 shall remain in full
force and effect throughout the term of the Agreement and any renewals thereof.
2. That the "Urinary Drug Screens (UDS)" chart in Section 2, "Non -Occupational Injury Health Care
Services," of Exhibit B, "Schedule of Compensation," is hereby amended to add the following:
