AMENDMENT
In accordance with the Professional Services Agreement signed between Matrix
Resources, Inc. ("Contractor") and the City of Fort Worth ("Client") on June 22, 2005,
City Secretary Contract (CSC) No. 32032, as amended by CSC No. 37230, collectively
the "Professional Services Agreement," Appendix B-5, which is incorporated into the
Professional Services Agreement is hereby amended to read as follows:
1. The term of this work order shall be extended to end when all services have
been completed under this work order as contemplated herein, unless
otherwise terminated in accordance with CSC No. 32032, as amended.
2. This Work Order incorporates by reference all of the terms and conditions of
the Professional Services Agreement, CSC No. 32032, as amended, and
specifically the services to be provided under Section 1 of said agreement.
3. The services to be performed under this Work Order shall be as follows:
Desktop Technician
To install PC hardware and peripheral components on the City's enterprise data
network; to load and verify correct operation of all application software packages
used throughout the City's data network, such as operating system, word
processing, spreadsheet programs, proprietary departmental software as well as
enterprise wide software; to provide training and technical assistance to users; to
identify and resolve hardware, software, and operator problems; to make minor
repairs, or refer to service personnel. Works as a resource on project
implementation; to serve as a liaison between assigned information systems
division, City personnel and departments, and outside agencies; to assist City staff
with various aspects of information systems; to assess department needs and
recommend equipment and software to purchase. To recommend appropriate
action to maintain current and up to date information systems; to maintain
awareness of new developments in the field of information technology; to
incorporate new developments as appropriate into programs; and to recommend
new equipment and software to purchase.

Payment Terms: OFMCIAL RECORD
