CONTRACT NO.
AMENDMENT No. 4
TO CITY SECRETARY CONTRACT
WHEREAS, the City of Fort Worth (CITY) and Transystems Corporation
Consultants, (ENGINEER) made and entered into City Secretary Contract No.
38678, (the CONTRACT) which was authorized by M&C C-23535 on the 19th
day of May, 2009 in the amount of $ 111, 700 . 00; and
WHEREAS, the CONTRACT was subsequently revised by: Amendment Number
1 in the amount of $6, 260 . 00 which was administratively authorized on
January 11, 2010, Amendment Number 2 in the amount of $4, 160 . 00 which was
administratively authorized on June 22, 2010, Amendment Number 3 in the
amount of $28, 666 . 00 which was administratively authorized on September
27, 2011; and
WHEREAS, the CONTRACT involves engineering services for the
following project:
Pavement Reconstruction and Water and Sanitary Sewer Main
Replacement on Hawthorne Avenue, 12th Street and Lisbon Street 2008
CIP Contract 9C, Project No. 01238; and
WHEREAS, it has become necessary to execute Amendment No. 4 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:

Article I of the CONTRACT is amended to include the additional
engineering services specified in a proposal dated April 5th, 2012, a
copy of which is attached hereto and incorporated herein. The cost to
City for the additional design services to be performed by Engineer
totals $10,803. 00.
2 .
Article II of the CONTRACT is amended to provide for an increase in
the fee to be paid to Engineer for all work and services performed under
the Contract, as amended, so that the total fee paid by the City for all
work and servic 11 be an amount of $161 ,589. 00.

All other provisions of the Contract, which are not expressly
amended herein, shall remain in full force and effect.

Dear Mr. Buckley,
Per your request,this amendment revises the previously approved Capital Improvement Project—Contract
9C, City Secretary Contract No. 38678 design plans and contract documents. This contract has been
amended by the following amendments:
Amendment No. 1 $ 6,260.00 Administrative Approval 1/11/2010
Amendment No. 2 $ 4,160.00 Administrative Approval 6/22/2010
Amendment No. 3 $28,666.00 Administrative Approval 9/27/2011
Specifically,Amendment No.4 includes preparation for pre-construction public meeting,verifying project
control and construction phase services as needed. This work is to be included with and will augment the
designs associated with the original contract. The specific work to be included is as follows:
• 1P Street, Hawthorne Avenue and Lisbon Street —
o Prepare and attend pre-construction public meeting
• Verify, mark and reset project control(Gorrondona and Associates)
• Construction phase assistance(as needed)
