COUNTY OF TARRANT §
WHEREAS, the City of Fort Worth (City) and Stream Water Group, Inc.
(Engineer) made and entered into City Secretary Contract No. 38894 (the Contract)
which was authorized by the City Council by M&C C-23647 on the 29th day of July,
2009 in the amount of$215,053.00, and;
WHEREAS, the Contract involves engineering services for the following project:
Engineering Design Services: Street reconstruction,water and sanitary sewer replacement on 16th St
NW(Clinton Ave to Circle Park Blvd),22nd St NW(Prairie Ave to McKinley Ave),Belmont Ave(NW 19th
St to NW 20th St),Odd St(Belmont Ave to Grand Ave),Gould Ave(NW 14th St to NW 16th St),and
Highland Ave(NW 18th St to Odd St).2008 CIP Project,Council District 2,Contract 2A;Project No.01320,
DOE No.6178
WHEREAS, it has become necessary to execute Amendment No.1 to said
Contract to include an increased scope of work and revised maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through their duly
authorized representatives, enter into the following agreement, which amends the
Contract:
1.
Article I of the Contract is amended to include the additional engineering services
specified in a proposal letter dated March 14, 2012 of which is attached hereto and
incorporated herein. The cost to City for the additional services to be performed by
Engineer total $7,990.00.
2.
Article 1I of the Contract is amended to provide for an increase in the maximum
fee to be paid to Engineer for all work and services performed under the Contract, as
amended, so that the total fee paid by the City for all work and services shall not exceed
the sum of$223,043.00
3.
All other provisions of the contract, which are not=O�FFICIAL ein, shall Ong
remain in full force and effect.

Dear Mr.Sahu:
The following is a summary of additional work requested for Amendment No. 1 to the above referenced project,per
recent design meetings with City of Fort Worth officials.
Amendment No. 1:
• Update pavement design sheets and cross section sheet to include side walk improvements
1. Updated 15 plan and profile sheets to include new sidewalk and replacement of existing sidewalk per
public meeting and City request
2. Revised 26 sheets to update cross-section for Sidewalk addition
Amendment No. 1 Amount(see Exhibit B-313):
Street Improvements(T&PW Department): $7,990.00
Total Amendment No.I (C204-531200-2024001320,Work Phase: 30) $7,990.00
The current status of the contract between Stream Water Group,Inc and City of Fort Worth for the above referenced
project is as follows:
Original Contract Amount= $215,053.00
Amendment No. 1 Amount= $ 7,990.00
Total Contract Amount including Amendment No. 1 = $223,043.00
